摘要 目的:探讨Ⅳ型胶原酶在胆管癌浸润和转移中的意义,以期了解胆管癌浸润和转移机理。方法:用免疫组化方法和图像分析系统观察胆管癌原发灶和转移灶标本切片癌细胞Ⅳ型胶原酶的表达。结果:Ⅳ型胶原酶免疫反应物呈棕色,弥漫性地分布于细胞浆内。正常胆管及胆囊腺瘤均无阳性反应物存在。无转移的原发灶切片癌细胞呈弱阳性染色反应,合并转移的原发灶切片和转移灶切片呈强阳性染色。转移灶及合并转移的原发灶切片免疫反应物平均光密度显著大于无转移的原发灶切片。结论:Ⅳ型胶原酶是胆管癌浸润和转移过程中参与降解细胞外基质的重要蛋白酶之一,提示未来在抗转移治疗中通过抑制Ⅳ型胶原酶及其它水解酶的活性,有可能获得显著的进展。 Objective: To explore the significance of collagenase type IV in the invasion and metastasis of cholangiocarcinoma to clarify the possible mechanism of the invasion and metastasis of cholangiocarcinoma. Methods: The expression of collagenase type IV were determined in the primary foci and metastatic foci of changiocarcinoma with immunohistochemistry and image analysis. Results: The immunoreactive product of collagenase type IV was stained brown and widely distributed in the cytoplasm of the cancer cells but not in the epithelium of the normal bile duct or adenoma. The immunostaining intensity was weak in the primary foci without metastasis but strong in the metastatic ones and primary ones complicated with metastasis. The mean immunostaining intensity was significantly stronger in metastatic foci and primary ones complicated with metastasis than in the primary foci without metastasis. Conclusion: Collagenase type IV is one of the important proteinases to degrade the extracellular matrix during the invasion and metastasis of cholangiocarcinoma, which suggests that great achievement can be made in anti-metastasis treatment of cholangiocarcinoma in the future through inhibiting the activity of collagenase type IV and other proteinases.
