狼疮肾炎患者尿MCP-1测定的临床意义 被引量:1

摘要 【目的】观察狼疮肾炎(LN)患者尿单核细胞趋化蛋白-1(MCP-1)浓度与LN活动的关系,试图寻找一种反映LN损害程度及疾病活动情况的非创伤性临床指标。【方法】选择本院确诊系统性红斑狼疮(SLE)及LN的患者40例,其中狼疮活动期患者18例,狼疮静止期患者22例,选择本院健康体检者20例作为正常对照组,采用双抗体夹心酶联吸附实验(ELISA)法检测患者晨尿MCP-1浓度,采用对-硝基苯酚比色法测定尿N-乙酰-B-D氨基葡萄糖酶(NAG),双缩尿法测定患者24 h尿蛋白定量,常规方法检测血尿肌酐浓度。【结果】LN患者尿MCP-1浓度明显高于正常对照组,狼疮活动期患者尿MCP-1浓度明显高于静止期患者,且尿MCP-1浓度与NAG浓度及尿蛋白定量成正相关(r分别为0.781和0.611,P均<0.01)。【结论】LN患者尿MCP-1浓度明显高于正常对照组,且与疾病的损伤程度和疾病的活动成正相关,尿MCP-1浓度可作为反映LN损害程度的指标。 [Objective] To review the relationship of urine concentration of monocyte chemoattractant protein-1 (MCP-1) and the activity of lupus nephritis(LN) and to try to find non-invasive clinical parameters reflecting the degree of injury of LN and activities of the disease . [Methods] Urine concentration of MCP-1 was measured by sandwich Elisa in 40 patients with lupus nephritis and 20 age- and sex-matched normal controls. [Results] The level of urine MCP-1 in patients was found to be significantly higher than that in normal controls. In LN patients, the level of MCP-1 was markedly increased in patients with active disease compared with those with inactive disease. There was a strong positive correlation between MCP-1 and NAG or urine protein concentration (r:0. 781,0. 611, P 〈0. 001). [Conclusion] The level of MCP-1 is helpful to judge disease activity of LN.
