摘要
目的：评价甘油保存角膜对角膜溃疡穿孔实行治疗性角膜移植术的临床疗效。方法：对79例（79眼）角膜溃疡穿孔患者，应用甘油冷冻保存角膜分别行板层角膜移植联合层间填垫术50例（50眼），行穿透性角膜移植术治疗29例（29眼），随访时间为6～24mo，观察术后视力、角膜植片透明度变化、植片排斥和感染复发等并发症的发生情况。结果：79例患者，75例治愈，保存了眼球，成功率为95％；6例真菌性溃疡穿孔患者，4例因合并眼内炎用药物不能控制或难治性青光眼而行眼内容摘出或眼球摘除术，2例术后角膜炎复发，二次手术治愈。结论：采用甘油冷冻保存角膜片对角膜溃疡穿孔者行治疗性角膜移植术，可及时去除病灶、保存眼球，为再次增视性角膜移植术创造条件。
AIM： To explore the clinical results of therapeutic keratoplasty using the donor corneas freezing-preserved with glycerine for treating corneal perforation.METHODS： A total of 79 eyes with corneal perforation underwent lamellar keratoplasty combined with interlamellar padding （50 eyes） or penetrating keratoplasty （29 eyes） with the glycerine freezing-preserved corneas. Patients were followed up for 6 to 24 months. Postoperative visual acuity, corneal graft transparency, graft rejection, recurrent infection and other complications were observed. RESULTS： After the operation, 75 cases were cured and eyeballs were retained. The success rate was 95%. Among 6 patients with fungal corneal ulcer, 4 cases were performed evisceration or enucleation owing to complicated endophthalmitis （medication couldn＇t control） or refractory glaucoma; 2 cases had recurrent keratitis and were cured by the second surgery. CONCLUSION： Therapeutic keratoplasty using donor corneas freezing-preserved with glycerine for treating corneal ulcer perforation may eliminate disease foci in time, keep the eyeballs and create conditions for next keratoplasty enhancing visual acuity.
