PCNA、Ki-67、CyclinD1、P53在泪腺上皮性肿瘤的表达及临床相关性研究

The expression and clinical significance of PCNA, Ki-67, CyclinD1 and P53 in epithelial tumors of lacrimal gland

摘要 目的 探讨PCNA、Ki-67、CyclinD1、P53的表达与泪腺上皮性肿瘤的发生、发展的关系及其临床病理意义。方法 应用Envision法检测1985年1月至2005年10月间存档的45例泪腺良、恶性上皮性肿瘤和6例正常泪腺组织中PCNA、Ki-67、CyclinD1、P53的表达,并将检查结果结合临床进行比较。结果 在正常泪腺组织组、泪腺多形性腺瘤初次手术组、泪腺多形性腺瘤复发组、泪腺多形性腺癌组、腺样囊性癌组中各免疫因子的阳性细胞比例分别为:PCNA:34.83%±12.64%、52.19%±9.42%、56.20%±9.28%、82.0%±8.46%、89.29%±5.24%、Ki-67:3.83%±4,62%、6.0%士6.51%、7.7%士8.35%、15.08%±11.41%、16.86%±12.27%、Cylin D1:5,50%±5.96%、13.25%±12.69%、12.1%±10.95%、15.17%±11.92%、17.140/0±11.49%;P53:7.13%±7.57%、8.5%±7.11%、26.33%±9.44%、22.29%±6.05%。经统计学分析,在泪腺良性上皮性肿瘤与恶性上皮性肿瘤之间比较均有统计学意义的有PCNA、Ki-67、P53;在正常泪腺与恶性上皮性肿瘤之间表达有统计学意义的有PCNA、P53;PCNA、Ki-67、CyclinD1、P53在初发和复发的泪腺多形性腺瘤中的表达差别无统计学意义。结论 PCNA、P53可作为鉴别泪腺上皮性肿瘤良恶性的参考指标。 Objective To investigate the relationship of PCNA, CyclinD1, Ki-67 and P53 protein expression in epithelial tumors of lacrimal gland with it's clinical and pathological characterisites. Methods The tissue chips of 45 cases of lacrimal tumor tissues from 1985 to 2005 in Changzheng hospital were analyzed by the immunohistochemical method (Envision's) compared with 6 cases of normal lacrimal gland tissues. Result The ratios of postive cells of each immunity genes in the group of normal lacrimal tissues were: PCNA: 34.83% ± 12.64%, Ki-67: 3.83% ±4.62%, CyclinDl: 5.50%± 5.96%, P53: none; in the group of pleomorphic adenoma without recurrence were: PCNA: 52.19%± 9.42%, Ki-67: 6.0%±6.51%, CyclinD1: 13.25%± 12.69%, P53: 7.13% ± 7.57%; in the group of pleomorphic adenoma with recurrence were: PCNA: 56.20% ± 9.28%, Ki-67: 7.7% ± 8.35%, CyclinDl: 12.1% ± 10.95%, P53: 8.5% ± 7.11%; in the group of pleomorphic adenocarinoma were: PCNA: 82.0% ± 8.46%, Ki-67: 15.08% ±11.41%,CyclinDh 15.17%± 11.92%, P53: 26.33% ± 9.44%; in the group of adenoid cystic carcinoma were: PCNA: 89.29v ± 5.24%, Ki-67: 16.86% ± 12.27%, CyclinDl: 17.14% ± 11.49%, P53: 22.29%± 6.05%. The expression of PCNA, Ki-67, and P53 were singificantly different between benign lacrimal epithelial tumors and malignant epithelial tumors (p〈0.01), there were no statistic defferences of PCNA, Ki-67, CyclinD1 and P53 expressions between the pleo- morphic adenoma without recurrence group and pleomorphic adenoma with recurence group. Conclusion the high expression of PCNA, Ki-67, CyclinD1 and P53 may be closely related to the occurrence, development and prognosis of epithelial tumors of lacrimal gland, the PCNA and P53 can be used as an index to identify malignant epithelial tumor.
