摘要
背景与目的:平滑肌肌动蛋白(smooth muscle actin,SMA)在神经系统检测中一般用来了解肿瘤的血管增生状态,其中偶然见到脑膜瘤有SMA阳性表达的细胞团,故本研究试探讨平滑肌肌动蛋白在脑膜瘤中表达的意义。方法:在常规神经病理诊断中66例脑膜瘤组织切片作HE和免疫组化染色:CK、EMA、Vim、SMA和CD34。免疫染色用EnVision二步法,光镜观察结果。结果:在66例脑膜瘤中,组织学分型依次为脑膜皮细胞型36例、纤维型17例、过渡型3例、不典型4例、其他良恶性类型共6例。66例中脑膜瘤细胞SMA阳性表达共16例(占24.24%),其中脑膜皮细胞型12例(75%)、纤维型3例、不典型型1例。结论:脑膜瘤细胞呈现SMA阳性表达,提示该细胞出现肌性分化(或去分化),拟称为肌脑膜皮细胞(myo-meningotheal cells),这种肿瘤可能成为一种新类型的脑膜瘤。
BACKGROUND & OBJECTIVE: To investigate the expression of the smooth muscle actin (SMA) immunoexpression in meningeoma. METHODS: Sixty-six meningeoma were routinely H.E processed and immunostain of CK, EMA, Vim, SMA and CD34 were performed by EnVision methods. RESULTS: In 66 cases of meningeoma, 21cases were male and 45 cases were female. The age of patients and the locations of the tumors distributed as the common. The histological types showed as follows: 36 cases were meningothelial, 17 fibrous, 3 transitional, 4 atypical and other 6 cases included anaplastic, psammomatous, angismatous,microcystic, lymphoplasmacyte-rich and mixed types. Neoplastic cells showed positive for SMA in 16 cases (24.24%), in which 12 cases were meningothelial (75%), 3 cases were fibrous and 1 case was atypical. The meningioma cells differentiated to angio-museular celLs(also called myo-meningotheal cells) instead of myo-epithelial cells is a form of mesenchymal differentiation. There is still no such histological classification for this subgroup of tumor.. CONCLUTIONS: Meningiomas ceils with SMA expression suggested its muscular differentiation (or dedifferentiation) and it is supposed to name it myo-meningothelial cells. This tumor type could probably become a new subtype in meningioma.
