摘要
目的:使用螺旋CT观察快速扩弓(RME)前后上颌牙列位置在三维方向上的变化。方法:使用螺旋CT获得临床患者颅面部三维数据模型,通过颅部的固有解剖标志点建立测量三维坐标系,使矫治前后的变化都在同一个测量坐标系下直接对比研究。观测10例恒牙早期患者在快速扩弓前后上颌牙列各标志点间的位置和角度改变。结果:1)后牙在宽度方向上为整体移动和倾斜移动相结合的移动方式,但第一磨牙和第一双尖牙稍有差别;牙齿和骨对宽度增加的贡献各占约1/2;2)后牙在垂直方向有伸长的趋势;3)第一磨牙和第一双尖牙均向前整体移动,轴倾度保持不变;4)中切牙整体随上颌骨前部向前向下移动,其牙长轴发生直立,轴倾度增加;5)上颌平面发生顺时针旋转。结论:上颌牙列位置RME前后在矢状向、垂直向和水平向3个方向上均发生了相应的改变。螺旋CT可以对这种三维位置变化进行详细研究。
Objective: To observe three -dimensional effect of upper arch after rapid maxillary expansion(RME) using spiral CT. Methods: 10 cases of RME patient were observed using spiral CT and a three -dimensional reference frame was set up to compare the position and angle changes of upper arch. Results: 1 ) Upper first molars and first premolars moved approximately in the same style. 2) Upper central incisors moved forward and downward together with maxillary, at the same time they upright in the sagittal plane and tip mesially. 3) The occlusion plane rotated clockwise for about 1.5°. 4) The width increasement effect of RME comes from approximately the equal amounts of skeletal and dental expansion. The mode of dental movement was a combination of body and tipping movements of posterior teeth. Conclusion: Upper arch and teeth move and incline in three - dimensional after RME treatment. Spiral CT can be used in the study of three - dimension changes of orthodontics research.
