摘要
目的观察慢速扩大上颌牙弓治疗功能性下颌偏斜的临床效果。方法选择本院口腔科门诊2005年1月～2009年5月收治的混合牙列和早期恒牙期患者19例,采用慢速扩大上颌牙弓方法治疗。结果扩弓前后两侧Cs、Go至ANS距离差异比较:治疗后Cs-ANS距离差(两侧髁突顶点和下颌角点至前鼻棘中点的线距差)、Go-ANS距离差(下颌骨两侧对应结构的线距差)减小,两组相比均有统计学意义(P<0.01),说明偏斜度降低。治疗后Cs-Go长度差(即两侧升支长度差)比治疗前略有下降,但差异无统计学意义(P>0.05),Cs-Me长度差(即下颌综合长度差)治疗后下降明显,治疗前后差异有统计学意义(P<0.05),Go-Me长度差(颌体长度差)治疗后下降明显,治疗前后差异有统计学意义(P<0.05)。结论功能性下颌偏斜,采用扩大上颌牙弓的方法,可以使下颌骨的不对称得到一定改善,是早期纠正下颌功能性偏斜的有效手段。
Objective To observe the slow expansion of maxillary arch treatment of functional clinical effect of mandibular deviation. Methods 19 patients with the mixed dentition and early permanent dentition were admitted to the dental clinic from January in 2005 to May in 2009,which treated with slow expansion of maxillary arch method. Results Before and after palatal expansion on both sides of Cs,the distance between ANS compared to Cs-ANS distance after treatment difference reduced（both sides of the mandibular condyle and the corner vertices to the midpoint of the anterior nasa] spine line length difference）,Go-ANS distance difference （mandibular on both sides of the line length difference corresponding structure） decreases, the two groups were statistically significant compared （P〈 0.01）, the skew decreased. After treatment,the length of Cs-Go difference （ie both sides of the ramus length difference）decreased slightly than before treatment,but the difference was not statistically significant（~0.05）. C s-Me length difference（ie,the length of the mandibular integrated SD） and Go-Me length difference （jaw body length difference）decreased significantly after treatmen,which were significantly before and after treatment （P〈0.05）. Conclusion The functional mandibular deviation, using expansion of maxillary arch can improve mandibular asymmetry,it is the early correction of functional mandibular deviation in the effective means.
