摘要
目的:探讨新辅助化疗前增殖细胞核抗原(PCNA)和P53在口腔鳞癌组织中的表达水平及其对卡铂联合5-氟脲嘧啶化疗方案(CF方案)敏感性预测的价值。方法:采用SP免疫组化法,对50例T2～T4口腔鳞癌患者用CF方案行2疗程新辅助化疗前后PCNA和P53的表达水平进行检测,分析其表达水平与化疗疗效的关系。结果:①化疗前口腔鳞癌中PCNA和P53的表达水平与化疗疗效显著相关(P<0.05);②化疗前PCNA高表达同时P53表达阴性患者行CF方案化疗有效率为90.00%,显著高于其它表达类型(P<0.05),PCNA低表达同时P53表达阳性的患者,CF方案化疗的有效率为14.29%,显著低于其它表达类型(P<0.05)。结论:化疗前PCNA和P53的表达水平均可作为预测口腔鳞癌对CF化疗方案敏感性的分子标记物,但用PCNA和P53两项指标的表达水平来联合预测显著高于单项指标预测。
Objective：To investigate the expression of p^53and proliferating cell nuclear antigen （PCNA）in oral squamous cell carcinoma （ OSCC ）and its chemosensitive predictive value to neoadjuvant chemotherapy（NACT） with Carboplatin and 5-Fu for the patients with OSCC. Methods：50 patients with T2 - T4 OSCC were examined by using SP immunohistochemistry before and after two NACT courses, the correlation between expression level of PCNA and p^53 and therapeutic effect was analyzed. Results： ①Expression level of PCNA and p^53 was significantly associated with response to chemotherapy（ P 〈0.05 ）; ② The effective rates of PCNA high expression and p^53 negative expression in OSCC before NACT was 90.00%, it was significantly higher than that of other expression types（ P 〈0.05 ）,and the effective rates of PCNA low expression and p^53 positive expression was 14.29%, it was significantly lower than that of other expression types （P 〈0.05）. Conclusion： Expression level of both PCNA and p^53 can be used as a chemosensitive predictive marker to neoadjuvant chemotherapy with Carboplatin and 5-Fu for the patients with OSCC, and combined analysis of PCNA and p^53 expression has higher predictive value than that of either PCNA or p^53.
