摘要
目的研究幽门螺杆菌(Hp)感染及其相关性疾病中过氧化酶体增殖因子活化受体γ(PPAR-γ)、环氧化酶2(COX-2)的表达及其相互关系以探讨 Hp 的致病机理。方法采用二步法免疫组化方法检测了209例内镜活检的胃黏膜内 COX-2、PPAR-γ的表达。209例包括:浅表性胃炎(CSG)75例(其中 Hp 阳性50例、Hp 阴性25例),萎缩性胃炎(CAG)24例(其中 Hp 阳性13例、Hp阴性11例),肠化与异型增生(IM+DYS)90例(其中 Hp 阳性45例、Hp 阴性45例)以及20例 Hp 阴性组织学大致正常的胃黏膜。同时还检测了36例 Hp 持续感染10年以上的 CSG 患者10年前后胃黏膜内 COX-2、PPAR-γ的表达。结果 (1)从对照组→CSG→CAG→IM+DYS,腺细胞内的 COX-2表达逐渐较强,阳性率分别为20%、42.7%、25.8%和61.1%(P<0.01或 P<0.05);炎症细胞中COX-2阳性率分别为0、32%、29.2%和36.7%,各疾病组均高于对照组(均 P<0.05)。(2)CSG 组及 IM+DYS 组 Hp 阳性者腺细胞和炎细胞中 COX-2的表达均显著高于 Hp 阴性者(P<0.01或 P<0.05)。(3)从对照组→CSG→CAG→IM+DYS,腺细胞内 PPAR-γ阳性率分别为0、18.7%、29.2%和45.6%(P<0.01或 P<0.05);炎细胞内 PPAR-γ阳性率分别为0、22.5%、20.8%和22.2%(各疾病组间比较,P>0.05)。(4)炎症细胞中 PPAR-γ表达在 CSG 组、IM+DYS 组 Hp 阳性者均显著高于Hp 阴性者(P<0.01);腺细胞中 PPAR-γ的表达在 CSG 组、IM+DYS 组 Hp 阳性者均显著高于 Hp 阴性者(P<0.05)。(5)Hp 持续感染的 CSG 患者,炎症细胞内 COX-2的表达10年后显著低于10年前(P<0.05),而炎症细胞内 PPAR-γ的表达10年前后差异无统计学意义(P>0.05)。(6)Hp 持续感染的 CSG 患者,10年后腺细胞内 COX-2、PPAR-γ的表达均显著高于10年前(P<0.01)。(7)Hp 阴性的各疾病组,炎细胞内 COX-2和 PPAR-γ的表达相互间均无相关性(r=0.006和0.149,均 P>0.05);在腺细胞内它们的表达在 IM+DYS 组呈显著正相关(r=0.336,P<0.05),其余各疾病组COX-2和 PPAR-γ的表达均无相关性(r=
Objective To investigate the expression of peroxisome proliferators-activated receptor-γ, (PPAR-γ) and cyclooxygenase-2 (COX-2) in Helicobacter pylori (Hp)- associated gastric mucosal lesions and the relations among them. Methods Two-step immunohistochemical staining was used to detect the expression of PPAR-γand COX-2 proteins in the gastric biopsy specimens from 209 patients with lesions of gastric mucosa, including 75 cases of chronic superficial gastritis ( CSG ) , 24 cases of chronic atrophic gastritis (CAG) , and 90 cases of intestinal metaplasia (MI) and dysplasia (DYS) , and from 20 subjects with normal gastric mucosa negative in Hp as controls. In addition, 36 CSG patients were followed up for 10 years to observe the changes of expression of PPAR-γ, and COX-2. Results ( 1 ) The expression of COXo2 protein in the glandular cells of the CSG, CAG, and IM + DYS groups were 42.7%, 45.8% , and 61.1% respectively, all significantly higher than that of the control group ( 20% , P 〈 0. 01 or 0. 05 ). The expression of COX-2 protein in the inflammatory cells of the CSG , CAG, and IM + DYS groups were 32% , 29.2% , and 36.7% respectively, all significantly higher than that of the control group (0% , all P 〈 0.05 ). (2) In the CSG and IM + DYS groups the expression levels of COX-2 in both the inflammatory and glandular cells were both significantly higher in the Hp-positive patients than in the Hp-negative patients (P 〈 0.05 or 0.01 ). (3) The expression of PPAR-γ, protein in the glandular cells of the CSG, CAG, and IM +DYS groups were 18.7%, 29.2%, and 45.6% respectively, all significantly higher than that of the control group ( 0%, all P 〈 0.01 ) ; and the expression levels of PPAR-γ protein in the inflammatory cells of the CSG, CAG, and IM + DYS groups were 22.5%, 20. 8%, and 22.2% respectively, all significantly higher than that of the control group (0%, P 〈0.05 or 0.01 ).
