摘要
目的探讨脑梗死与颈动脉粥样硬化之间的关系。方法对117例脑梗死患者进行颈动脉彩色多普勒超声检查,93例非脑梗死患者为对照组。观察梗死侧和非梗死侧,脑梗死组与对照组血管异常发生率、斑块类型及分布情况并进行比较。结果脑梗死组颈动脉内膜增厚及斑块形成等血管异常发生率为85·5%(100/117),显著高于对照组(45·2%,P<0·01),两组粥样硬化斑块检出率分别为65·8%(77/117),32·3%(30/93),差别有显著意义(P<0·01)。颈动脉粥样硬化斑块多发生在脑梗死病灶的同侧,占76·5%。以颈总动脉分叉处居多,其次为颈内动脉近段。脑梗死组患者软斑及复合性斑块的发生率高于对照组。结论脑梗死的发生与颈动脉粥样硬化,尤其是粥样硬化斑块密切相关,颈动脉超声对防止斑块形成、预防脑梗死的发生有重要意义。
Objective To explore the correlation between cerebral infarction and carotid arteriosclerosis. Methods Color Doppler ultrasonagraphy of the carotid artery were performed on 117 patients with cerebral infarction （the observation group） and 93 people without cerebral infarction were chosen as controls. The abnormal appearance of carotid artery, the number and position of plaques at both the infarction and infarction - free side were observed and compared with the controlled group. Results Of 117 patients, 100（85.5% ） patients show abnormal appearance of carotid artery. The incidence in observation group was significantly higher than control group （45.2 %, P 〈0. 01 ）. The occurrence rates of carotid arteriosclerosis plaques was 65.8 % （77/117） in observation group and 32.2% （30/93） in control group respectively. There are significantly difference between them. Most of the carotid plaques occurred in the same side of stroke focus （76.5%）. And most of the plaques existed in the bifurcation common carotid artery, followed by proximal internal carotid artery. The occurrence rates of soft plaques and complex plaques in case group were higher than in control group. Conclusion The occurrence of stroke is closely associated with the carotid arteriosclerosis, especially arteriosclerotic plaques. Ultrasonography of carotid artery is very important for find the formation of plaques and for avoiding the occurrence of stroke.
