摘要
目的探讨常用的一次性聚氯乙烯输液器对硝酸甘油、酚妥拉明和利多卡因等3种抢救药物的吸附作用及其影响因素。方法用紫外分光光度计对输液前与输液中各样品的药物浓度进行测定,观察一次性聚氯乙烯输液器对3种药物有无吸附作用和吸附程度;观察输液时间、速度以及药物浓度等3种因素对吸附作用的影响规律。结果①聚氯乙烯输液器对硝酸甘油吸附后,药物浓度显著下降,与吸附前相比有明显差异,具有统计学意义(P<0.01);对酚妥拉明吸附后,药物浓度轻度下降,与吸附前相比差异具有统计学意义(P<0.01);对利多卡因吸附作用不明显,方差分析差异无统计学意义(P>0.05)。②聚氯乙烯输液器对硝酸甘油的吸附程度与输液速度成反比、与药物浓度和输液时间成正比。在输液的8h内,累计吸附百分比可达52%。③聚氯乙烯输液器对酚妥拉明的吸附程度与输液速度无关,与药物浓度成正比。结论一次性聚氯乙烯输液器对利多卡因无吸附作用;对酚妥拉明的吸附作用较小;对硝酸甘油的吸附作用较强,已经影响到了有效治疗剂量,应引起护理人员的高度重视。硝酸甘油不适宜长时间维持输液,否则可增大吸附作用,增加吸附量。科学合理地选择硝酸甘油输液速度和药物浓度,可使吸附程度降到最低。
Objective To explore the adsorption of some first-aid drugs such as nitroglycerin, phentolamine and lidocaine onto infusion system made of polyvinyl chloride. Methods By assaying the samples concentration before and during transfusion with the ultraviolet spectrometer, we observed the drug adsorption and the influences of flow rates, drug concentrations and durations of drug infusion through intravenous giving sets made of polyvinyl chloride. Results There was no evidence of absorption of lidocaine by polyvinyl chloride. Concentrations of nitroglycerin and phentolamine were decreased via the intravenous giving sets made of polyvinyl chloride, with statistic significance (P〈0.01). Highest adsorption rates of nitroglycerin occured when the flow rates were low, nitroglycerin concentrations were high, and the infusion durations were long. The adsorption rates of phentolamine were related to the concentration and were higher at beginning. Conclusions Nitroglycerin was readily adsorbed onto intravenous giving sets made of polyvinyl chloride. There was no adsorption of lidocaine by polyvinyl chloride. The adsorption of phentolamine by polyvinyl chloride was slight. Nurses should pay attention to the infusion flow rate and drug concentration administration when nitroglycerin is instilled with intravenous giving sets made of polyvinyl chloride.
