卡维地洛对慢性心力衰竭患者血浆脑钠肽水平的影响 被引量:4

摘要 目的观察卡维地洛对慢性心力衰竭患者血浆脑钠肽水平的影响。方法将123例慢性心力衰竭患者随机分为卡维地洛组(62例)和对照组(61例)。治疗后观察脑钠肽和超声心动图的结果。结果卡维地洛组较对照组治疗后血浆脑钠肽水平显著降低(P<0.05);卡维地洛组治疗后左心室射血分数(LVEF)、每搏量以及每分排血量明显增加(P<0.05,P<0.01);卡维地洛组治疗前后患者血浆脑钠肽的下降水平与LVEF增加呈负相关(△LVEF:r=-0.86,P<0.01)。结论卡维地洛能明显改善慢性心力衰竭患者血流动力学,抑制神经内分泌因素的过度激活,改善心功能;血浆脑钠肽水平可作为评价β受体阻滞剂治疗慢性心力衰竭疗效的监测指标之一。 Objective To observe the effects of carvedilol on plasma level of brain natriuretic peptide(BNP) in patients with chronic heart failure(CHF). Methods 123 cases of CHF patients were randomly divided into two groups, the carvedilol group( n = 62) and conventional group( n = 61). The changes in level of BNP and echocardiographic indices were evaluated after 6 months. Results Level of BNP was significantly decreased in carvedilol group, LVEF, SV and CO were significantly increased in carvedilol group compared with conventional group. Meanwhile, changes in BNP level were negatively correlated with those in LVEF( r = - 0.86, P 〈 0.01). Conclusions Carvedilol may stabilize hemodynamics, suppress neurohormonal activation, and improve cardiac function in CHF patients. Plasma BNP level may serve as a therapeutic indicator for treatment of CHF patients.
