摘要
目的:探讨冠心病患者不同心功能级别Tei指数和脑钠肽(BNP)的变化及其相关性。方法:按照NYHA心功能分级将入选者分为4组:选取冠脉造影正常排除冠心病心功能Ⅰ级者(20例)为对照组(A组);心功能Ⅱ级者为B组,心功能Ⅲ级者为C组,心功能Ⅳ级者为D组,每组20例,B、C、D组均为经冠脉造影证实至少一支主要冠状动脉狭窄大于70%的冠心病患者;超声心动图测定左室射血分数(LVEF)、二尖瓣口舒张早期与舒张晚期血流峰值速度比值(E/A)、Tei指数;同期取血检测BNP浓度。结果:随着心功能级别的升高Tei指数与BNP逐渐增加(P<0.05),4组的Tei指数分别为0.40±0.04、0.50±0.05、0.62±0.08、0.78±0.07;4组的BNP分别为(44.62±15.21)ng/L、(197.45±41.84)ng/L、(410.70±278.18)ng/L、(774.75±371.96)ng/L;4组LVEF分别为67.52±4.54、64.85±5.40、51.85±9.73、43.50±8.46,A、B2组差别无统计学意义,余各组间差别有统计学意义(P<0.05);4组E/A分别为1.06±0.27、0.83±0.20、0.80±0.19、0.78±0.31,A组与B、C、D组差别均有统计学意义(P<0.05),后3组之间差别无统计学意义。Tei指数与BNP呈正相关(r=0.90),LVEF与BNP呈负相关(r=-0.68),E/A与BNP无线性关系。结论:Tei指数与BNP存在较好的相关性,是一种无创、简便、敏感、准确、综合评价心脏整体功能的新指标。
Objective:To investigate the correlation between the Tel index and BNP and their changes in the coronary artery disease patients with different cardiac function. Methods: Twenty patients with normal heart function were in group A. Sixty patients with coronary artery disease who were diagnosed by coronary artery angiography were divided into 3 groups by the degree of heart failure. Twenty patients with NYHA grade Ⅱ were in group B, 20 patients with NYHA grade Ⅲ in group C, and 20 patients with NYHA grade Ⅳ in group D. The diagnostic criteria of coronary artery disease was that there was a stenosis more than 70% in left main artery or one main branch artery at least. Left ventricular ejection fraction (LVEF), E/A, and Tei index were measured by echocardiography. Plasma BNP was assayed at the same time. Results: Tei index and BNP increased with the heart dysfunction aggravated (P 〈 0.05). Tei index of the 4 groups were 0.40±0.04, 0.50+0.05, 0.62±0.08, and 0.78±0.07 respectively. BNP of the 4 groups were (44.62±15.21) ng/L, (197.45±41.84) ng/L,(410.7±278.18) ng/L, and (774.75±371.96) ng/L respectively. There were significant differences in any two groups (P 〈 0.05). LVEF of the 4 groups were 67.52±4.54, 64.85±5.4, 51.85±9.73, and 43.50±8.46 respectively. There was no significant difference in group A and B, and there were significant differences in the other groups (P 〈 0.05). E/A of the 4 groups were 1.06±0.27, 0.83± 0.20, 0.80±.19, and 0.78±0.31 respectively. There were significant differences in group A and group B, C and D (P 〈 0.05), while, there was no significant differences in group B, C and D (P 〉 0.05). There was a significant negative correlation between LVEF and BNP (OR= -0.68, P〈 0.05). There was also a significant positive correlation between Tel index and BNP (OR = 0.90, P 〈 0.05).
