Expression of C-kit and SCF in human cholangiolocellular carcinoma and its significance

摘要 目的拟通过检测 C-kit、SCF 基因在胆管细胞癌中的表达来了解 C-kit、SCF 与胆管细胞癌发生和发展之间的关系。方法采用免疫组化 S-P 法检测48例肝胆管细胞癌及20例肝外胆管癌标本 C-kit、SCF 蛋白表达情况,同时取对应的癌旁胆管组织及另取30例肝及胆管良性病变胆管组织作对照,所有数据采用 X^2检验。结果 48例肝胆管细胞癌组织中 C-kit、SCF 蛋白表达分别为39例(81.3%)和36例(75.0%),20例肝外胆管癌组织中 C-kit、SCF 蛋白表达分别为12例(60.0%)和13例(65.0%),癌旁胆管组织中4例 C-kit、5例 SCF 呈弱阳性表达,30例肝及胆管良性病变胆管组织 C-kit、SCF 蛋白均呈阴性表达。两癌组织与癌旁胆管组织、肝及胆管良性病变胆管组织 C-kit、SCF表达阳性率比较差异有显著性(P<0.05)。Ⅲ~Ⅳ期胆管细胞癌 C-kit、SCF 蛋白表达阳性率高于Ⅰ~Ⅱ期(P<0.05),中低分化癌高于高分化癌(P<0.05),有淋巴结转移癌高于无淋巴结转移癌(P<0.05)。而肝外胆管癌组织中 C-kit、SCF 蛋白表达与其临床病理各因素无明显相关(P>0.05)。C-kit、SCF 蛋白表达与胆管细胞癌病人年龄、性别、肿瘤直径无关。结论 C-kit、SCF 蛋白过表达可能在胆管细胞癌的发生、发展中发挥作用。 Objective To investigate the correlation of expression of C-kit and SCF with carcinogenesis and tumor progression of human cholangiolocellular carcinoma (HCCC). Methods SP immunohistochemistry was used to detect the expression of C-kit and SCF in 48 cases of HCCC, 20 cases of extrahepatic cholangiocarcinoma and corresponding paracancerous bile duct tissues and 30 cases of benign liver and bile duct disease. The results were processed with X^2 test. Results The positive expression of C-kit and SCF was detected in 39 cases (81.3%) and 36 cases (75.0%), respectively, for 48 cases of HCCC, 12 (60.0%) and 13 (65.0%) for 20 cases of cholangiocarcinoma. Only 4 cases and 5 had poor expression in paracancerous bile duct tissues. No expression of C-kit and SCF was found in the 30 cases of benign liver and bile duct disease. The expression level of C-kit and SCF was significantly higher in the 2 groups of patients with malignancy than in those with benign liver and bile duct disease (P〈0. 05). The expression of C-kit and SCF in HCCC was significantly higher in Ⅲ-Ⅳ stage than in Ⅰ-Ⅱ stage (P〈0. 05), in immediately and poorly differentiated carcinomas than in highly differentiated ones (P〈0. 05) and in carcinomas with lymph node metastasis than in those without (P〈0. 05). However, the expression of C-kit and SCF in extrahepatic cholangiocarcinoma was not closely related to clinicopathological features. C-kit and SCF were not correlated to age, sex and tumor size in patients with HCCC. Conclusions The over-expression of C-kit and SCF may play an important role in occurrence and progression of HCCC.
