结直肠癌微血管密度与螺旋CT灌注成像的相关性 被引量:8

Correlation between microvessel density and spiral CT perfusion imaging in colorectal carcinoma

摘要 目的探讨结直肠癌微血管密度与螺旋CT(SCT)灌注成像的相关性。方法经手术和病理证实的结直肠癌患者37例,水灌肠后行SCT扫描。先行结直肠肿块范围的平扫,选定靶平面后再进行同层动态增强扫描,采用東芝Xpress/SX螺旋CT机自带的灌注功能软件,绘制所选层面的癌灶、靶动脉的感兴趣区的时间密度曲线(TDC)。根据TDC计算病灶的血流灌注量(PF)。癌组织切片经鼠抗人CD34单克隆抗体进行免疫组织化学染色,测定癌组织微血管密度(MVD),将结直肠癌的MVD与血流灌注量进行比较。结果结直肠癌MVD为[87.28±41.18(33.11~173.44)]条/高倍视野,PF为[39.74±10.82(15.60~64.80)]ml·min-1·100g-1。MVD、PF在Dukes分期、浆膜浸润、淋巴结转移比较中差异均无统计学意义(P值均>0.05);MVD、PF随着病理分期的期别升高均有下降趋势,但两者无显著相关性(r=0.18,P>0.05)。结论结直肠癌的MVD与其血流灌注量之间无明显相关性,结直肠癌早期存在高度血管化和高的血流灌注量,SCT灌注成像可能较MVD更好地反映肿瘤的微循环功能。 Objective To evaluate the correlation between microvessel density (MVD) and spiral CT (SCT) perfusion imaging in colorectal carcinoma. Methods Thirty-seven patients with histologically proven colorectal carcinoma underwent water enema spiral CT scan. The largest axial surface of the primary tumor was searched on unenhanced spiral CT images, at this level the enhanced dynamic scan series was acquired. Time-density curves (TDC) were created from the region of interest drawn over the tumor and target artery by Toshiba Xpress/SX spiral CT with perfusion functional software. Then the perfusion was calculated. MVD was evaluated by using immunohistochemical staining of surgical specimens with anti- CD34, and then MVD was correlated with perfusion. Results MVD of colorectal carcinomas was from 33. 11 to 173.44, mean 87.28, and perfusion was from 15.60 to 64. 80 ml·min^-1·100g^-1, mean 39.74 ml·min^-1·100g^-1. Both MVD and perfusion were not associated with invasive depth, metastasis and disease stage (P 〉 0.05 ), and they all decreased with increasing Dukes' stage, but no significant correlation was found between them ( r = 0. 18, P 〉 0.05 ). Conclusion There is no significant correlation between MVD and perfusion. Neovascularizaton and perfusion are highly presented in early colorectal carcinoma. SCT may be more suitable for assessing tumorigenesis than histological MVD technique.
