兔肝门静脉阻断的MSCT血管成像评价 被引量:3

Evaluation of multi-slice CT angiography in detecting portal vein occlusion in rabbits

摘要 目的探讨MSCT的肝动脉与门静脉血管成像价值,评价MSCT血管成像(mu lti-slice CT angiograph,MSCTA)在显示兔肝门静脉阻断效果中的作用。方法20只新西兰大白兔行门静脉左外支结扎,另20只仅行假手术对照。3周后行MSCTA检查,对每只实验兔的动脉期和门静脉期图像进行容积再现(volum e rendering,VR)、最大密度投影(m aximumintensity projection,M IP)及多平面重建(mu ltip lanar reform ation,MPR)。并统计其肝动脉与门静脉的Ⅰ、Ⅱ级分支及≥Ⅲ级分支的显示情况,尤其是门静脉的阻断情况。结果肝左外叶门静脉分支结扎者均能被MSCTA检查证实。Ⅲ级以上肝动脉分支的MSCTA显示率(40%)明显低于Ⅰ、Ⅱ级肝动脉分支的显示(分别为100%和70%)(P<0.05)。对于门静脉分支的MSCTA显示方面,各级分支的显示率无显著性差异(P>0.05)。结论MSCTA可无创直观地显示兔肝动脉与门静脉的空间解剖细节,在兔肝门静脉阻断效果的评价中起着重要作用。 Objective To study the value of multi-slice CT angiography (MSCTA) in detecting the hepatic vascular anatomy and to evaluate the efficacy of portal vein occlusion in rabbits. Methods The portal branch ligation (PBL) of the left extemal branch of portal vein was carried out in 20 New Zealand white rabbits, and sham operation in another 20 rabbits. Three weeks later, MSCT plain scan and MSCT three-phase enhanced scan were performed respectively on these rabbits by GE lightspeed ultra 16 scanner. Both arterial and portal phase 3D vascular imaging was processed by using volume rendering (VR) , maximum intensity projection (MIP) and multiplanar reconstruction (MPR) technique. The displaying rate of Ⅰ , Ⅱ, Ⅲ or above branches of hepatic arteries and portal veins was calculated respectively, especially for that in the region of PBL. Results PBL could be confirmed by MSCTA. The total displaying rate of the Ⅲ and above branch of hepatic arteries by MSCTA (40%) was significantly lower than that of Ⅰ and Ⅱ branches of hepatic artery ( 100% and 70% , respectively) ( P 〈 0. 05 ) , but there was no statistical difference on the displaying rate of the branches of portal vein (P 〉 0. 05). Conclusion MSCTA can clearly display spacial details of the hepatic vascular anatomy in rabbits. As a harmless examination method, MSCTA plays an important role in post-operative evaluation of the portal vein occlusion in rabbits.
