摘要
目的探讨乌司他丁(Ulinastatin,UTI)对细菌性严重脓毒症大鼠外周血各细胞因子的表达的水平以及中性粒细胞(polymorphonuclear neutrophil,PMN)凋亡情况的影响。方法将SD大鼠随机分为5组:正常对照组、病理对照组、生理盐水治疗对照组、氧氟沙星抗感染组、乌司他丁-氧氟沙星抗炎抗感染组,每组10只。经腹腔内注射创伤弧菌建立脓毒症模型,15 h后取单侧颈总动、静脉血标本测定IL-10、TNF-αI、L-1β,锌金属蛋白酶,溶细胞素的水平以及在不同时间测定PMN的凋亡情况。结果在严重脓毒症大鼠的确存在PMN凋亡滞后的现象,合用乌司他丁的抗炎抗感染组较单纯氧氟沙星抗感染组及生理盐水对照组IL-10水平显著增高(P<0.01),而TNF-αI、L-1β,锌金属蛋白酶,溶细胞素水平则显著降低(P<0.01)。中性粒细胞凋亡时间明显提前。结论乌司他丁能改善严重脓毒症大鼠的预后,其机制可能通过调节各细胞因子的分泌水平并改变PMN凋亡滞后的现象。从而减轻组织器官炎症反应的病现损害而起保护作用,避免全身炎症反应综合征进一步向MODS发展。
Objective To investagate the effect of ulinastation on the level. of cytokines in peripherial blood and apoptosis of polymorphonuelear ueutrophil(PMN) with severe pyemia in the rats. Methods Dividing SD rats into five groups randomly, norml control group, pathologic eonparision group; saline cure group, ofloxacin anti-infection group; ulinastation combined with ofloxation group; 10 in every group. Vibrio vnlnificus pyemia rats model was established through injecting vibrio vnlrificus into intraperiton, adoptting the single common carotoid artery, vein after 15 h to measure the level of IL-10, TNF-α、IL-1β, zinc-metaUoendopoptidases, cytokines and dertermine the apoptosis of PMN in different time. Results Apoptosis ratio decreases significantly in severe pyemia rats and could be increased by ulinastation, combined ulinastation with ofloxation can pregulate IL-10 and downregulate TNF-α、IL-1β, zinc-metalloendopoptidases, cytokines, at the same time, it can bring forwaxd the time of apopotis of PMN compared otloxation can group and saline group. Conclusion Ulinastation can change the prognosis of the rat. The mechanism is perhapars regulating the level of cytokines and the lag of PMN apaptosis. Protect the tissue and cell, alleviate the inflammation reaction and pathologic lesion of tissue and organ, avoid the SIPS to develop to MODS.
