多药耐药基因在原发性肝癌的表达及其临床意义 被引量:16

摘要 目的探讨多药耐药基因(MDR)在原发性肝癌中的表达及其临床意义。方法取肝细胞癌标本64例,逆转录聚合酶链反应(RTPCR)法半定量检测肝癌多种多药耐药基因的基因表达情况,分析其临床意义。结果(1)各多药耐药基因的表达率高,各耐药基因的表达率差异无统计学意义。(2)各多药耐药基因的表达量差异无统计学意义。(3)MDR1、多药耐药相关蛋白(MRP)、谷胱甘肽S转移酶(GST)π和肺耐药蛋白(LRP)的mRNA表达量有随分级而增高的趋势;DNA拓扑异构酶(TOPOⅡ)的mRNA表达量有随分级而下降的趋势。(4)肝癌MDR基因mRNA表达量与肿瘤大小、有无转移、有无包膜和结节数目有关,与AFP水平、肝硬化情况、肝功能状况无关。(5)在有效组中MDR1、MRP、GSTⅡLRPmRNA的表达量低于无效组;TOPOⅡmRNA的表达量有效组高于无效组,差异无统计学意义。结论在肝癌多药耐药基因mRNA的表达率高;在肝癌多药耐药的基因mRNA的表达与肿瘤大小、有无转移、结节数目、有无包膜相关,与化疗的疗效相关。 Objective To investigate the expressions of multidrug resistant (MDR) genes in HCC, and analyze its clinical significance. Methods The expressions of MDR genes of tumor tissues of 64 eases and adjacent tissues of 12 eases of HCC were detected with RT-PCR. Results (1)The expression rates of MDR1,MRP, GST-π, LRP, TOPOⅡ mRNA was lingh, and there was no statistical difference among them. (2)there was no statistical difference among the expression level of MDR's. (3)The expression level of MDR1, MRP, GST-π, LRP mRNA had a incline to rise as HCC stage become higher, The expression level of TOPOⅡ had a trend to decline as HCC stage become higher. (4)The expression level of MDR genes had positive correlations with tumor volume, tumor nodules, tumor metastasis, and had negative correlation with tumor encapsulation, and showed no significant correlation with the level of AFP, the degree of cirrhosis and the function of liver. (5)In the group of chemotherapy response, the expression level of MDR1, MRP, GST-π, LRP mRNA were lower than that in'the group of chemotherapy non-response, the expression level of TOPOⅡ mRNA in the group of chemotherapy response was higher than that in the group of chemotherapy non-response. Conclusion There were high MDR genes express rates in HCC. The expression level of MDR had correlation with the stage of HCC and the result of chemotherapy, had positive correlation with tumor volume, tumor nodules, tumor metastasis.
