摘要 目的探讨缺氧诱导因子-1α(HIF-1α)、血管内皮生长因子(VEGF)表达对人脑胶质瘤预后的影响.方法采用免疫组织化学方法检测63例脑胶质瘤HIF-1α、VEGF的表达状况,并将结果与临床预后随访结果进行综合分析.结果HIF-1α总阳性表达率为71.4%,其中强表达16例(25.4%),中度表达14例(22.2%),弱表达15例(25.4%),不表达18例(28.6%);与正常对照组比较有显著性差异(P<0.01).HIF-1α表达水平与胶质瘤恶性程度相关(r=0.467,P<0.01).HIF-1α主要表达于肿瘤边缘或与正常组织交界处的细胞及坏死的肿瘤组织中.不同组织学分级的脑胶质瘤标本肿瘤细胞质均有VEGF表达,胶质瘤Ⅰ级、Ⅱ级、Ⅲ~Ⅳ级VEGF阳性细胞数分别为175±66、352±100、662±123(个),3组比较有显著性差异(P<0.01).VEGF与HIF-1α表达水平有关(r=0.817,P<0.01).患者生存时间与HIF-1α呈负相关(r=-0.824,P<0.01).结论VEGF与HIF-1α表达有一定的相关性,HIF-1α与脑胶质瘤新生血管生成有关.HIF-1α、VEGF联合检测可更准确判断患者的预后. Objective To study the expressions of HIF-1α and VEGF in gliomas and their correlations with prognosis. Methods The expression of HIF-1α and VEGF was investigated by immunohistochemistry in 63 specimens of gliomas. Then the influence of HIF-1α expression on survival was investigated. Results Strong expression of HIF-1α was observed in 16(25.4%), moderate in 14(22.2%), and weak in 15(23.8%), and no expression was found in 18(28.6%). There are statistical significance in groups(P<0.01). Expression for HIF-1α increased as pathological grade(r=0.467,P<0.01). The positive cells were mainly found in tumor cells infiltrating the brain at the tumor margin and necrotic zones. Every grade gliomas can express VEGF and there are statistical significance in groups (P<0.01). the date of positive cells of VEGF in glioma gradeⅠ, glioma gradeⅡ and glioma grade Ⅲ-Ⅳ was 17566?352100?662123 respectively. VEGF was significantly correlated with expression level of HIF-1α(r=0.817,P<0.01). The surrival time was significantly correlated with expression level of HIF-1α(r=-0.824,P<0.01). Conclusion The expression for VEGF may be correlated with HIF-1α. HIF-1α is correlated with the novel ngiogenesis of the gliomas tumors. The expressions of HIF-1αand VEGF can be used to accurately assess the prognosis in glioma patients.
