摘要
目的:了解不同年龄段儿童特应性皮炎(AD)的临床特征。方法:分析了156例儿童AD临床资料,并按不同年龄段进行比较和统计。结果:79%AD患儿于出生后1年内发病。1岁以内组和1～2岁组皮损主要表现为急性及亚急性湿疹样损害,好发于头面部及四肢伸侧;3～6岁组和7岁以上组大多数表现为慢性苔藓样斑块样皮损,好发于四肢屈侧,并常累及肘窝和窝。AD患儿中寻常型鱼鳞病、白色糠疹和毛周角化症的发病率与对照组相比差异无显著性(P>0.05),而干皮症、面部苍白、耳下裂纹和弥漫性头皮屑的发病率与对照组相比差异有非常显著性(P<0.01)。结论:儿童AD的皮损形态及好发部位随年龄增长而出现明显变化。干皮症、面部苍白、耳下裂纹和弥漫性头皮屑在儿童AD的诊断中可能具有特异性。
Objective:To study the clinical features in the children with atopic dermatitis(AD)in different age.Meth ods:One hundred and fifty-six children with AD were enrolled for this10-month investigation.The types,distribution of skin le-sions,asso ciated clin ical man ifesta tions,and laborato ry findings were studied and compared.Results:The lesions of AD ap-peared dur ing the first year of life in75%children.In both groups of under1year of age and1to2years of age,the a-cute and subacute eczematous lesions were predominant,which mostly distributed on the face,scalp and the extensive surface of extremi ties.In3to6years group and elder than7years of age,the chronic flexural lichenoid lesions were more pre sent,which com monly affected an tecubital and popliteal areas.The incidence of ichthyosis vulgaris,keratosis pilaris and pityriasis alba in AD patients was not significantly high er(P>0.05)than these in control group,whereas the incidence of xero sis,fa-cial pallor,chronic scalp scaling and infra-auricular fissures was sig nificantly higher than these in control group(P<0.01).Con clu sions:The types and distribution of skin le sions in children AD varied with age.Xerosis,fa cial pallor,chronic scalp scaling,infra-auric ular fis sures might be specific for the diagnosis of AD.
