摘要
目的 研究周围神经在扩张延长过程中保留扩张囊对扩张延长神经功能的影响。方法 在预实验的基础上 ,用特制的组织扩张器将家兔坐骨神经近段扩张延长 4 0 % ,在扩张延长结束后分别于第 2d和第 30d取出扩张器 ,观察扩张延长神经的血循环、神经功能 (神经传导速度、肌肉复合动作电位和胫前肌湿重 )以及组织结构和超微结构的变化 ,并进行计算机图像分析。结果 所有扩张延长神经的延长率均达到 4 0 % ,延长结束后第 2d即取出扩张器组较保留扩张器 30d取出组 ,坐骨神经的微血管分布稀少 ,神经传导速度、动作复合电位、胫前肌湿重、神经纤维数量、髓鞘形状等均有明显差异 ,后者优于前者。结论 在一定范围内 ,神经在扩张延长过程中造成的神经损害是可逆的 ,这种损害可以通过扩张延长器的留置时间而得以恢复。
Objective This experiment was designed to study the effect on the function of the expanded nerve by retaining the expander.Methods On the basis of pre-experiment,the rabbit sciatic nerves were expanded slowly for 16 days with a tissue expander designed by ourselves in pre-experiment.After the nerves having been elongated by 40%,the tissue expanders were taken away from beneath the elongated nerves on the 2nd day and the 30th day respectively.The changes of microcirculation,physiological function,histology and ultrastucture of the expanded nerves were observed systematically.Results All the nerves were elongated by 40% after removing of the expanders,the microcirculation,physiological function,histology and ultrastructure in the 30th day group were better than that of 2nd day group.Conclusion The injuries of the nerve during expansion,of course up to a point,were reversible and can be alleviated by retaining the tissue expander after the expansion having been finished.
