摘要
目的 探讨 5一Fu、HCPT腹腔化疗联合平消片口服对晚期肝癌的疗效。方法 6 5例不适宜手术及介入栓塞化疗的晚期肝癌病人 ,随机分 2组。实验组 :平消片长期口服 ,5一氟尿嘧啶 (5 -Fu) 75 0mg/m2 、羟基喜树碱 (HCPT) 10mg/m2 腹腔化疗 ,每周 1次 ,连用 2次 ,每 3周重复 ;对照组 :单纯给予护肝、对症、支持治疗。结果 实验组 :客观肝肿瘤缩小 6例 ,达PR 4例 (12 5 % ) ,CR 0例 ,NCl2例 (37 5 % ) ,PDl6例(5 0 0 % )。对照组 :无肝肿瘤缩小。两组腹水减少 1周以上者分别为 34 4 %、9% ,P <0 0 5。甲胎蛋白下降率分别为 31 3%、3% ,P <0 0 5。KPS评分增高率分别为 31 3%、9% ,P <0 0 5。差异均有统计学意义。血浆白蛋白回升率分别为 2 5 0 %、12 1% ,P >0 0 5 ,无统计学意义。中位生存期分别为 15周、7周。结论 低剂量 5 -Fu、HCPT腹腔化疗联合平消片口服治疗晚期肝癌不良反应轻 ,部分患者肿瘤缩小 ,生存质量提高 ,中位生存期明显延长。
Objective To evaluate the curative effect of intraperitoneal chemotherapy with 5-Fu and HCPT combining with taking orally Ping Xiao pian in advanced hepatic carcinoma.Methods Sixty five patients of advanced hepatic carcinoma,who could not be treated by surgical operation and transcatheter arterial chemoembolization,were subdivided randomly into two groups:① The treatment group: Keep on taking orally Ping Xiao pian.Intraperitoneal chemotherapy with 5-Fu 750mg/m 2 and HCPT 10mg/m 2. Once a week,for 2 times,every 3 weeks repeat;② The control group: treated with protecting liver, symptomatic,sustaining simply.Results The treatment group: The hepatic tumor objective contracted were 6 cases.PR were 4 cases (12.5%) .No CR, NC 12 cases (37.5%), PD 16 cases (50.0%). The control group:No shrinking tumor cases. In the two groups,abdomen water reduced for more than one week were respectively 34.4% and 9%,P<0.05. The rate of AFP descending were 31.3% and 3%,P< 0.05 .The improvement rate of KPS score were 31.3% and 9%,P<0.05. All were statistically significant difference. The rate of blood plasma albumin rising were 25.0% and 12.1%,P>0.05. There was no statistically significant difference. The median surviral period were respectively 15weeks and 7weeks.Conclusion The poisonous and side reaction of intraperitoneal chemotherapy with low amount of 5-Fu and HCPT combining with taking orally Ping Xiao pian in advanced hepatic carcinoma were not serious.The tumor were shrinking in some patients. The quality of life was enhanced. The median surviral period was obviously lengthened.
