摘要
目的:分析γ干扰素体外释放酶联免疫检测技术(ELIIGRA)的临床应用价值。方法:以本院年龄大于等于15岁的住院患者1002例为研究对象,包括培养阳性肺结核患者294例,培养阴性肺结核患者296例,非结核患者412例。收集研究对象临床资料,ELIIGRA结果和病原学检测结果,通过统计学分析评价ELIIGRA的临床应用价值并探讨其影响因素。结果:以临床诊断作为"金标准"来评价ELIIGRA,该技术的灵敏度为83.7%(508/607),特异性为79.2%(313/395),约登指数为0.629,阳性预测值为0.861,阴性预测值为0.759。年龄增大ELIIGRA的敏感性会下降,初治肺结核患者的敏感性高于复治的肺结核患者。结论:ELIIGRA检测技术作为结核分枝杆菌感染的筛查试验不适宜临床单独应用来诊断或排除肺结核患者,应当结合临床资料加以综合的分析。另外应用的过程中要重视年龄和抗结核治疗等因素造成的影响。
Objective The arm of this study was to investigate the clinical usefulness of Interferon-Gamma Release by enzyme-linked immunosorbent Assay(ELIIGRA)in the diagnosis of pulmonary tuberculosis.Method We conducted an investigation in our hospital to evaluate the role of ELIIGRA in the diagnosis of pulmonary tuberculosis.All cases underwent the investigation are old than or equal to 15 years,here we collected the clinical information and pathogen test data to evaluate the performances of ELIIGRA in a diverse patient population,including patients tuberculosis cultured positive(n=294),tuberculosis cultured negative(n=296)and non-tuberculosis(n=412).Results The sensitivity,specificity,Youden index,positive predict value,negative predict value of ELIIGRA is equal to 83.7%(508/607),79.2%(313/395),0.629,0.861,0.759 respectively,using the clinical diagnosis as"golden standard".The age and the history of tuberculosis treatment are the risk factors associated with ELIIGRA.Conclusions ELIIGRA,as a screening test for pulmonary tuberculosis with its inadequate sensitivity and specificity,limit the usefulness as a single test to rule-in or out pulmonary tuberculosis.We recommend that clinical data should be analyzed comprehensively.In addition,we should pay attention to the effects of factors such as age and anti-tuberculosis treatment when we use ELIIGRA.
