3.0 T MR IDEAL-IQ序列在慢性冈上肌肌腱损伤中脂肪浸润的定量评估价值 被引量:10

Quantitative Study of 3.0 T MR IDEAL-IQ Sequence in Chronic Supraspinatus Tendon Injury

摘要 目的采用3.0 T MR IDEAL-IQ技术定量测量冈上肌脂肪分数(FF)值,并分析冈上肌FF值与冈上肌肌腱损伤程度(TTS)、年龄、性别、身体质量指数(BMI)以及症状持续时间的关系。方法选取157例有肩关节临床症状的患者或志愿者,进行肩关节MRI常规序列扫描及IDEAL-IQ序列斜矢状位扫描。按照常规MRI上TTS表现分为正常组、退变组、部分撕裂组和完全撕裂组。通过后处理软件测量冈上肌FF值,并对冈上肌FF值与冈上肌TTS及受试者特征进行统计学分析。结果在冈上肌完全撕裂组、年龄≥51岁、BMI≥30 kg/m^2、女性和较长的症状持续时间的亚组中可以观察到较高的冈上肌FF值,冈上肌FF值与冈上肌TTS、年龄、性别及BMI之间存在相关性(r=0.84、0.59、0.21、0.34),但和症状持续时间无关(P>0.05)。结论IDEAL-IQ可以实现慢性冈上肌腱损伤中脂肪浸润的定量研究。 Objective To quantitatively measure supraspinatus muscle fat fraction(FF)using 3.0 T MR IDEAL-IQ imaging,and to analyze the relationship between FF and the degree of supraspinatus tendon tear severity(TTS),age,gender,body mass index(BMI)and symptom duration.Methods 157 patients or volunteers with clinical symptoms of shoulder joints were enrolled in this study,and underwent conventional MRI scanning of shoulder joints and oblique sagittal scanning of IDEAL-IQ sequences,and FF maps were obtained.The supraspinatus was graded into 4 groups based on routine MRI-findings to characterize the TTS:normal group,degeneration group,partial-thickness tear group,and complete tear group.Statistical methods were used to evaluate the relationship between FF,TTS and subject characteristics.Results Statistically higher supraspinatus fat fractions were observed in subgroups with complete tear group,age≥51 years,BMI≥30 kg/m^2,female gender,and longer symptom duration.The FF value of supraspinatus was correlated with TTS,age,gender and BMI of supraspinatus(r=0.84,0.59,0.21,0.34),but was not related to symptom duration(P>0.05).Conclusion IDEAL-IQ can be used to quantify fat infiltration in chronic supraspinatus tendon injury.
