常规磁共振与磁共振间接关节造影诊断肩袖损伤的对比研究 被引量:17

摘要 目的对比常规磁共振(MR)和MR间接关节造影诊断肩袖损伤的效能。方法对临床怀疑肩袖损伤的56例患者均行常规MR、MR间接关节造影检查及肩关节镜检查。以肩关节镜检查作为诊断"金标准",比较常规MR、MR间接关节造影检查对肩袖全层撕裂与部分撕裂、肌腱炎的诊断效能。结果 56例患者经关节镜检查确诊为肩袖损伤46例,其中,肩袖部分撕裂24例,全层撕裂16例,肌腱炎6例。常规MR与MR间接关节造影诊断肌腱炎的结果与关节镜检查结果一致,其诊断效能相当。常规MR与MR间接关节造影诊断肩袖部分撕裂的特异度均为95.5%,诊断全层撕裂的特异度分别为93.3%、100.0%。MR间接关节造影诊断肩袖部分撕裂的灵敏度91.7%、准确度95.7%均明显高于常规MR的37.5%、67.4%,差异有统计学意义(P<0.05)。常规MR与MR间接关节造影诊断肩袖全层撕裂的灵敏度、准确度均较好,分别为87.5%和95.7%、93.8和97.8%,两种方法灵敏度、准确度比较,差异无统计学意义(P>0.05)。结论常规MR检查和MR间接关节造影均是诊断肩袖损伤全层撕裂可靠而较为准确的检查手段;MR间接关节造影可明显提高诊断肩袖部分撕裂的灵敏度、准确度。 Objective To compared the efficacy of conventional MR with MR indirect arthrography in diagnosing rotator cuff injury.Methods A total of 56 patients with rotator cuff injury were performed with conventional MR,MR indirect arthrography,and shoulder arthroscopy( as diagnostic criteria),according to which,the efficacies of the former two methods was compared in diagnosing complete rotator cuff tears,partial rotator cuff tear,and tendonitis. Results Of all 56 patients with rotator cuff injury,24 were diagnosed by shoulder arthroscopy as partial rotator cuff tear,16 as complete rotator cuff tears,and 6 as tendonitis. In the diagnosis of tendonitis,both tested methods had equal diagnostic efficacy. In the diagnosis of both partial rotator cuff tear and complete rotator cuff tears,specificity of the two methods were of no significant difference( both 95. 5% in partial rotator cuff tear,and 93. 3% and 100. 0%,respectively in complete rotator cuff tear). In diagnosing partial rotator cuff tear,sensitivity( 91. 7% vs. 37. 5%) and accuracy( 95. 7% vs. 67. 4%) were both higher in MR indirect arthrography( P〈0. 05),while in diagnosing complete rotator cuff tear,neither sensitivity( 87. 5% vs. 93. 8%) nor accuracy( 95. 7% vs. 97. 8%) of the two methods was different( P〈0. 05). Conclusion Both conventional MR and MR indirect arthrography are reliable and accurate examines for the diagnosis of complete rotator cuff tears,while the later can obviously raise the sensitivity and accuracy in the diagnosis of partial rotator cuff tear.
