摘要
目的探究基于3.0MRI的非病灶性颞叶癫癎(nl-TLE)患者脑白质网络结构变化与执行功能的关系。方法回顾性分析在2018年1月至2020年8月于宣城市人民医院就诊的112例nl-TLE患者的临床资料,另选择112例健康者为对照组,比较两组受试者的脑白质网络结构以及执行功能,并行Pearson相关性分析。结果与对照组比较,nl-TLE组脑白质体积占全脑体积的百分比、数字广度顺向分数、词语流畅性个数以及BNT分数更低(均P<0.05),Stroop C耗时更高(P<0.05)。与对照组比较,nl-TLE组的左扣带回海马部、右下纵束、左穹隆以及右穹隆的FA值更低(均P<0.05),左下额枕束以及左钩束的平均弥散系数(MD)值更高(P<0.05)。数字广度顺向与左穹隆各项异性分数(FA)值呈正相关(r=0.481,P=0.047);Stroop C与右下纵束FA值呈负相关(r=-0.501,P=0.038),与左下额枕束MD值呈正相关(r=0.709,P=0.001);词语流畅性与左扣带回海马FA值(r=0.602,P=0.011)和右下纵束FA值呈正相关(r=0.668,P=0.003),与左下额枕束MD值(r=-0.537,P=0.024)和左钩束MD值呈负相关(r=-0.584,P=0.015);BNT与右穹隆FA值呈正相关(r=0.569,P=0.021),与左钩束MD值呈负相关(r=-0.634,P=0.009)。结论nl-TLE患者存在执行功能障碍,且执行功能障碍与脑白质网络结构改变有关。
Aim To explore the relationship between the changes of brain white matter network structure and executive function in patients with non-lesional temporal lobe epilepsy based on 3.0MRI.Methods The clinical data of 112 nl-TLE patients who were treated in Xuancheng People’s Hospital from January 2018 to August 2020 were retrospectively analyzed,and 112 healthy people were selected as the control group,and the white matter network structure and executive function of the two groups were compared.Correlation analysis was performed using Pearson.Results Compared with the healthy group,patients in the nl-TLE group had lower white matter volume percentages of the whole brain volume,digit span orthodox scores,word fluency counts,and BNT scores(P<0.05),and higher Stroop C time(P<0.05).Compared with the healthy group,the FA values of the left in the nl-TLE group were lower(P<0.05),and the MD values of the left inferior fronto-occipital tract and the left uncinate tract were higher(P<0.05).The digital breadth was positively correlated(r=0.481,P=0.047)with the FA value of the left dome.Stroop C was negatively correlated(r=-0.501,P=0.038)with the FA value of the lower right longitudinal tract,and was positively correlated(r=0.709,P=0.001)with the MD value of the left lower frontal occipital tract.Word fluency was positively correlated with the FA value of the hippocampus of the left cingulate gyrus(r=0.602,P=0.011)and the FA value of the right lower longitudinal tract(r=0.668,P=0.003),and was negatively correlated with the MD value of the left lower frontal occipital tract(r=-0.537,P=0.024)and the left uncinate tract(r=-0.584,P=0.015).BNT was positively correlated(r=0.569,P=0.021)with the FA value of the right fornix,and negatively correlated(r=-0.634,P=0.009)with the MD value of the left hook bundle.Conclusion Patients with non-focal temporal lobe epilepsy have executive dysfunction,and the patient’s executive dysfunction is related to changes in the structure of the white matter network.
