不同负荷室性期前收缩患者左心室几何构型、校正QT间期及NT-proBNP水平的比较 被引量:2

Comparison of left ventricular geometry, corrected QT interval and NT-proBNP level in patients with different loads of premature ventricular contraction

摘要 【目的】分析比较不同负荷室性期前收缩患者左心室几何构型、校正QT间期及N-末端B型利钠肽原(N-terminal proB-type natriuretic peptide,NT-proBNP)水平。【方法】选取2018年1月至2020年6月因室性期前收缩于本院心内科住院的177例患者为研究对象,根据室性期前收缩负荷数将患者分为低负荷组64例、中负荷组66例和高负荷组47例。采用酶联免疫吸附(ELISA)法检测血清NT-proBNP水平;采用超声诊断仪检测患者心功能参数室性期前收缩频次、QRS波时限、QT间期、校正QT间期等;采用Spearman法分析室性期前收缩负荷与相关指标的相关性。【结果】低负荷组、中负荷组及高负荷组左心室几何构型比例不同,且左心室离心性肥厚、正常重构患者比例比较,差异有统计学意义(P<0.05),并随着室性期前收缩负荷增加,左心室离心性肥厚患者比例增加,正常重构患者比例降低。高负荷组QRS波时限、QT间期、校正QT间期、NT-proBNP水平高于中负荷组和低负荷组,中负荷组QRS波时限、QT间期、校正QT间期、NT-proBNP水平高于低负荷组,差异有统计学意义(P<0.05)。室性期前收缩负荷与离心性肥厚、QRS波时限、QT间期、校正QT间期、NT-proBNP呈正相关,与正常重构呈负相关(P<0.05)。【结论】随室性期前收缩负荷增加,左心室离心性肥厚患者比例增加,QRS波时限、QT间期、校正QT间期时限延长,NTproBNP水平增加,室性期前收缩负荷与各指标均呈一定的相关性,应注意密切监测相关指标,及早干预。 【Objective】To analyze and compare the left ventricular geometry,corrected QT interval and N-terminal pro-B-type natriuretic peptide(NT-proBNP)level in patients with different loads of premature ventricular contraction.【Methods】A total of 177 patients with premature ventricular contraction who were hospitalized in the Department of Cardiology of our hospital from January 2018 to June 2020 were selected as the research objects.They were divided into low load group(64 cases),medium load group(66 cases)and high load group(47 cases)according to the load of premature ventricular contraction.Enzyme-linked immunosorbent assay(ELISA)was used to detect the level of serum NT-proBNP.Echocardiography was used to detect cardiac function parameters,including frequency of premature ventricular contraction,QRS duration,QT interval and corrected QT interval.Spearman method was used to analyze the correlation between load of premature ventricular contraction and related indexes.【Results】The proportions of left ventricular geometry in low load group,medium load group and high load group were different,and the proportions of patients with left ventricular centrifugal hypertrophy and normal remodeling were significantly different(P<0.05).With the load increase of premature ventricular contraction,the proportion of patients with left ventricular eccentric hypertrophy increased,and the proportion of patients with normal remodeling decreased.QRS duration,QT interval,corrected QT interval and NT-proBNP level in the high load group were higher than those in the medium load group and low load group,and QRS duration,QT interval,corrected QT interval and NT-proBNP level in the medium load group were higher than those in the low load group(P<0.05).Load of premature ventricular contraction was positively correlated with eccentric hypertrophy,QRS duration,QT interval,corrected QT interval and NT-proBNP,but negatively correlated with normal remodeling(P<0.05).【Conclusion】With the increase of ventricular premature systolic load,the
