摘要
目的探究成人变应性鼻炎患者治疗后生活质量变化及与治疗依从性的关系。方法回顾性分析我院在2017年1月至2019年1月期间确诊成人变应性鼻炎患者(共计100例)临床资料,采用鼻-结膜炎相关生活质量问卷(RQLQ)评估患者生活质量,治疗依从性调查问卷评估患者治疗依从性,根据患者有无接受用药指导将其分为甲组(接受用药指导,n=72)和乙组(未接受用药指导,n=28),有无接受过用药监督分为丙组(接受过用药监督,n=70)和丁组(未接受过用药监督,n=30),将复诊总次数>6次或者是复诊总时间>12个月记为治疗依从性好组(n=40),将复诊总次数<6次或者是复诊总时间<12个月者为治疗依从性差组(n=60),使用Spearman秩相关分析探究患者治疗仪从性和生活质量的关系。结果甲组患者日常活动受限、睡眠、非鼻-结膜炎症状、鼻炎相关行为、鼻部症状、眼部症状、心理情感反应评分均低于乙组(P<0.05),丙组日常活动受限、睡眠、非鼻-结膜炎症状、鼻炎相关行为、鼻部症状、眼部症状、心理情感反应评分低于丁组(P<0.05),治疗依从性差组患者活动受限、睡眠、非鼻-结膜炎症状、心理情感反应评分高于依从性好组(P<0.05);治疗依从性和RQLQ日常活动受限、睡眠、非鼻-结膜炎症状、鼻炎相关行为、鼻部症状、眼部症状、心理情感反应领域呈正相关(r_(s)=0.387,P=0.011;r_(s)=0.607,P=0.023;r_(s)=0.432,P=0.030;r_(s)=0.459,P=0.040;r_(s)=0.158,P=0.005;r_(s)=0.419,P=0.010;r_(s)=0.422,P=0.009)。结论成人变应性鼻炎患者治疗后的生活质量容易受较多因素影响,尤其是在治疗依从性方面和其接受的教育程度方面,两者和其生活质量有着密切关系,加强教育程度低的患者关注度及注重其治疗依从性的提高,对其在进行治疗后的生活质量能够产生更加积极的影响。
Objective:To explore the relationship between quality of life and treatment compliance in adult patients with allergic rhinitis after treatment.Methods:Clinical data of 100 adult patients diagnosed with allergic rhinitis in our hospital from January 2017 to January 2019 were retrospectively analyzed.The quality of life of the patients was assessed using the naso-conjunctivitis Related Quality of Life questionnaire(RQLQ),and the treatment compliance questionnaire was used to assess the treatment compliance.Patients were divided into group A(receiving medication guidance,n=72)and Group B(not receiving medication guidance,n=28)according to whether they had received medication guidance,and group C(receiving medication supervision,n=70)and group D(not receiving medication supervision,n=30).Patients with a total number of visits>6 times or a total time of visits>12 months were classified as a group with good treatment compliance(n=40),and patients with a total number of visits<6 times or a total time of visits<12 months were classified as a group with poor treatment compliance(n=60).Spearman rank correlation analysis was used to explore the relationship between therapeutic adherence and quality of life of patients.Results:The scores of daily activity restriction,sleep,non-naso-conjunctivitis symptoms,rhinitis related behaviors,nasal symptoms,eye symptoms and psychological emotional response in group A were lower than those in group B(P<0.05).The scores of limited daily activities,sleep,non-naso-conjunctivitis symptoms,rhinitis related behaviors,nasal symptoms,eye symptoms and psycho-emotional response in group C were lower than those in group D(P<0.05),and the scores of limited activities,sleep,non-naso-conjunctivitis symptoms and psychoemotional response in group C were higher than those in group D(P<0.05).Treatment compliance was positively correlated with RQLQ activity restriction,sleep,non-naso-conjunctivitis symptoms,rhinitis related behaviors,nasal symptoms,ocular symptoms,and psycho-emotional response domains(r_
