摘要
目的探讨在臂丛节后神经成像中施行MR3D-STIR SPACE序列增强扫描的临床价值及成像质量。方法选取50例、2021年1月-2022年8月臂丛神经患者,均行MR常规序列扫描、MR3D-SPACE-STIR序列平扫及增强扫描。对患者的损伤部位、形态、信号强度与近、远侧段臂丛神经的相关性,并对比平扫及增强图像质量。结果MRI平扫诊断节后神经损伤敏感性为89.47%、特异性为83.33%、准确性88.64%。增强MR3D-SPACE-STIR的组织对比显著提高,经处理后,可对臂丛神经节后的走行图像进行完整显示。且该技术增强序列图像的对比噪声比(CNR)为(0.57±0.09)、背景抑制评分(3.56±0.63)分及对臂丛神经显示评分(3.96±0.53)与平扫(0.35±0.06)、(1.58±0.51)分、(2.02±0.59)分均存在显著差异(t=12.231,12.214,10.170;P<0.05)。结论MR 3D-SPACE-STIR序列增强扫描的背景抑制效果较佳,可使臂丛神经损伤及走形得以清晰显示,其臂丛神经节后图像质量较平扫明显提升。
Objective To explore the clinical value and imaging quality of MR3D-STIR SPACE sequence enhanced scan in the postganglionic nerve imaging of brachial plexus.Methods 50 patients with brachial plexus from January 2021 to August 2022 were selected to undergo routine MR sequence scanning,MR3D-SPACE-STIR sequence plain scanning and enhanced scanning.The correlation between the injury site,shape,signal intensity and the proximal and distal brachial plexus of the patients was analyzed,and the image quality was compared with plain scan and enhanced image.Results The sensitivity,specificity and accuracy of MRI in the diagnosis of postganglionic nerve injury were 89.47%,83.33%and 88.64%,respectively.The tissue contrast of enhanced MR3DSPACE-STIR was significantly improved.After processing,the walking image of brachial plexus after ganglion could be completely displayed.In addition,the contrast noise ratio(CNR)of the enhanced sequence images was(0.57±0.09),the background suppression score(3.56±0.63),and the brachial plexus display score(3.96±0.53)was significantly different from that of plain scan(0.35±0.06),(1.58±0.51),and(2.02±0.59)(t=12.231,12.214,10.170;P<0.05).Conclusion The enhanced MR 3D-SPACE-STIR sequence has a better effect on background suppression,which can clearly show the brachial plexus injury and its shape,and its postganglionic image quality is significantly improved compared with that of plain scan.
