肌电生物反馈疗法联合运动疗法对分娩性臂丛神经损伤患儿肩手功能的影响

Effect of Electromyography Biofeedback Therapy Combined with Exercise Therapy on Shoulder and Hand Function in Children with Obstetric Brachial Plexus Palsy

摘要 目的探讨肌电生物反馈疗法联合运动疗法对分娩性臂丛神经损伤(OBPP)患儿肩手功能的影响.方法选取2018年5月—2022年1月我院收治的70例OBPP患儿为研究对象,根据随机数字表法将患儿分为对照组(n=35)和观察组(n=35).对照组采用运动疗法,观察组在对照组基础上采用肌电生物反馈疗法,均持续治疗12周.比较两组临床疗效、临床症状和肩手功能.结果观察组治疗总有效率94.29%高于对照组的74.29%,差异有统计学意义(P<0.05).治疗前,两组肢体麻痹、举升困难积分及Fugl-Meyer上肢功能评定量表(FMA-UE)评分比较,组间差异无统计学意义(P>0.05);治疗后,观察组肢体麻痹积分(1.13±0.25)分、举升困难评分(1.28±0.33)分均低于对照组的(1.75±0.32)分、(1.86±0.35)分,FMA-UE评分(48.58±4.29)分高于对照组的(41.47±2.36)分,组间差异有统计学意义(P<0.05).结论对OBPP患儿采用肌电生物反馈疗法联合运动疗法,能够有效改善患肢症状,增强临床疗效,促进肩手功能恢复. Objective To investigate the effect of exercise therapy combined with electromyography biofeedback therapy on the shoulder and hand function in children with obstetric brachial plexus palsy(OBPP).Methods 70 patients with OBPP admitted to our hospital from May 2018 to January 2022 were selected as the research objects,and divided into the control group(n=35)and the observation group(n=35)according to the random number table method.The control group was treated with exercise therapy,and the observation group was treated with electromyography biofeedback therapy on the basis of the control group,with both interventions for 12 weeks.The clinical efficacy,clinical symptoms and limb motor function were compared between the two groups.Results The total effective rate of treatment 94.29%of the observation group was higher than 74.29%of the control group,the difference was statistically significant(P>0.05).Before treatment,there were no significant differences in limb paralysis,lifting difficulty score and Fugl-Meyer assessment-upper extremity(FMA-UE)score between the two groups(P>0.05);after treatment,the limb paralysis score(1.13±0.25)points and lifting difficulty score(1.28±0.33)points in the observation group were lower than(1.75±0.32)points and(1.86±0.35)points in the control group,and the FMA-UE score(48.58±4.29)points was higher than(41.47±2.36)points in the control group,with statistically significant differences between the groups(P<0.05).Conclusion The combination of electromyography biofeedback therapy and exercise therapy for children with OBPP can effectively improve the symptoms of affected limbs,enhance the clinical efficacy,and promote the recovery of shoulder and hand functions.
