磁共振三维成像3D-SPACE-STIR、3D-SPACE-T2WI序列及图像融合技术对复杂性肛瘘的诊断价值 被引量:23

摘要 目的探讨三维可变反转角快速自旋回波短时间反转恢复(3D-SPACE-STIR)、3D-SPACE-T2WI序列及图像融合技术对复杂性肛瘘的诊断价值。方法回顾性分析复杂性肛瘘患者56例,术前均行磁共振3D-SPACE-T2WI、3D-SPACE-STIR序列扫描并进行图像融合。对图像清晰度评分,评估三组图像质量;进一步比较各序列和融合图所显示的内口、瘘管及分支,做出影像学诊断,以手术诊断结果为金标准,对比三者诊断效能。结果图像质量评价中,对括约肌的观察,3D-SPACE-T2WI序列和融合图较3D-SPACE-STIR成像质量更好;对于瘘管及其分支的成像,则3D-SPACE-STIR序列和融合图优于3D-SPACE-T2WI序列;括约肌、瘘管(分支)的综合评估,融合图评分最高。三种成像方式对内口和瘘管(分支)诊断的准确率分别为88.52%、78.68%、91.80%(χ2=4.818,P=0.09),84.61%、76.92%、92.31%(χ2=5.909,P=0.052),与手术比较无统计学差异。结论融合图在复杂性肛瘘中成像质量较好,对病灶整体显示具有优势。作为影像学辅助性手段,3D-SPACE序列及其融合图在复杂肛瘘的诊断中有较好的准确性和参考价值。 Objective To explore the diagnostic value of 3 D-SPACE-STIR,T2WI sequence and image fusion technology in complex anal fistula.Methods Retrospective analysis of 56 cases with complicated anal fistula was performed Methods A retrospective analysis of 56 patients with complicated anal fistula was performed with magnetic resonance 3 D-SPACE-T2WI and 3 D-SPACE-STIR sequence scans and image fusion before operation.Image sharpness for the 3 groups was scored for quality assessment by paired t test.The internal orifice and fistula with branches were counted in both sequence and fusion images.Imaging diagnosis was compared with surgical diagnosis for accuracy rate and the chi-square test was used to study statistic difference.Results Images in 3 D-SPACE-T2WI sequence and fusion showed better quality in observing the sphincter.3 D-SPACE-STIR sequence and fusion were better for fistula with branches.Fusion images displayed the best results than single 3 D-SPACE-T2WI or 3 D-SPACE-STIR sequence in comprehensive evaluation of the sphincter and fistula.Diagnostic accuracy rates of the three imaging methods for internal orifice and fistula with branches were 88.52%、76.68%、91.80%(χ2=4.818,P=0.09),84.61%、76.92%、92.31%(χ2=5.909,P=0.052),which showed no difference with surgical diagnosis.Conclusion Fusion images show better quality in complex anal fistula,especially in overall display of lesions.As an aid to diagnosis,3 D-SPACE sequence and its fusion images have better accuracy and reference value in identifying complex anal fistulae.
