摘要
目的探讨跨膜蛋白16A(TMEM 16A)表达与冻融胚胎移植(Freeze-thaw embryo transfer,FET)妊娠结局之间的相关性。方法选取2018年3月~2020年3月于我院首次进行FET治疗的160例患者作为研究对象,根据妊娠结局分为成功妊娠组106例和未妊娠组54例。在移植前一天通过微活检术取所有患者子宫内膜组织,采用实时荧光定量PCR(qRT-PCR)法检测并比较两组患者TMEM 16A以及白血病抑制因子(LIF,Leukemia?inhibitory?factor)mRNA表达水平,采用Western blot法检测两组患者的TMEM 16A以及LIF蛋白表达水平。并采用logistic多因素回归分析FET妊娠结局的独立影响因素。结果妊娠成功组患者的TMEM 16A mRNA、LIF mRNA表达水平高于相比未妊娠组,差异具有统计学意义(P<0.001)。妊娠成功组患者的TMEM 16A、LIF蛋白表达量高于未妊娠组,差异具有统计学意义(P<0.001)。Spearman相关性分析结果表明TMEM 16A以及LIF mRNA表达水平之间存在明显正相关性(r=0.658,P=0.000)。Logistic回归结果显示年龄(OR=2.787,95%CI=1.209~6.423)、TMEM 16A蛋白表达量(OR=0.506,95%CI=0.289~0.884)、LIF蛋白表达量(OR=0.502,95%CI=0.304~0.829)、移植胚胎评分(OR=0.417,95%CI=0.231~0.753)均是影响FET妊娠结局的独立危险因素。结论子宫内膜跨膜蛋白16A可能参与了子宫内膜容受性建立,其表达水平与冻融胚胎移植妊娠结局存在密切相关性。
Objective:To study the relationship between transmembrane protein 16A expression and pregnancy outcome of freeze-thaw embryo transfer.Methods:160 patients who received FET treatment for the first time in our hospital from March 2018 to March 2020 were selected as study subjects.According to pregnancy outcomes,they were divided into the successful pregnancy group(106 patients)and the non-pregnancy group(54 patients).The endometrial tissue of all patients was taken by micro biopsy the day before transplantation.The real-time fluorescence quantitative PCR was used to detect the TMEM 16A and LIF mRNA expression levels in both groups.The Western blot was used to detect the TMEM 16A and LIF protein expression levels in both groups.The logistic multivariate regression analysis was used to FET the independent influencing factors of pregnancy outcome.Results:The expression levels of TMEM 16A mRNA and LIF mRNA in the pregnant group were higher than those in the non-pregnant group,and the differences were statistically significant(P<0.001).The expression levels of TMEM 16A and LIF protein in the pregnant group were higher than those in the non-pregnant group,and the difference was statistically significant(P<0.001).Spearman correlation analysis showed that there was a significant positive correlation between TMEM 16A and LIF mRNA expression levels(r=0.658,P=0.000);Logistic regression results showed that age(OR=2.787,95%CI=1.209-6.423),TMEM 16A protein expression(OR=0.506,95%CI=0.289-0.884)and LIF protein expression(OR=0.502,95%CI=0.304-0.829)and transplanted embryo score(OR=0.417,95%CI=0.231~0.753)were independent risk factors for FET pregnancy outcome.Conclusion:Endometrial transmembrane protein 16 A may be involved in the establishment of endometrial receptivity,and its expression level is closely related to the pregnancy outcome of freeze-thaw embryo transfer.
