摘要
目的研究腔隙性脑梗死(LI)患者血清同型半胱氨酸(Hcy)、尿酸(UA)和超敏C反应蛋白(hs-CRP)水平并探讨其与颈动脉粥样硬化(CAS)程度的关系。方法选取LI患者90例,根据颈动脉内膜中层厚度(IMT)将患者分为斑块组29例、IMT增厚组38例和正常组23例,检测并比较各组血清Hcy、UA、hs-CRP水平及IMT值,分析Hcy、UA、hs-CRP与IMT的关系。结果LI患者血清Hcy、UA、hs-CRP水平及IMT值均高于健康人群(P<0.05);随着LI患者CAS程度增加,血清Hcy、UA、hs-CRP水平升高,各组差异均有统计学意义(P<0.05);LI患者血清Hcy、UA、hs-CRP水平与IMT值均具有明显正相关性(P<0.05);Hcy、UA和hs-CRP均是影响LI患者IMT的重要危险因素(P<0.05)。结论LI患者血清Hcy、UA、hs-CRP水平明显升高,且与颈动脉IMT存在明显正相关性,均是导致CAS严重程度增加的重要危险因素,并可能增加LI发病风险。
Objective To investigate the levels of serum homocysteine(Hcy),uric acid(UA)and hypersensitive C-reactive protein(hs-CRP)in patients with lacunar infarction(LI)and to explore their relationship with degree of carotid atherosclerosis(CAS).Methods Ninety patients with LI were selected.According to the carotid intima-media thickness(IMT),the patients were divided into plaque group(n=29),IMT thickening group(n=38)and normal group(n=23).The levels of serum Hcy,UA and hs-CRP and IMT value were detected and compared among the groups,and the relationship between the levels of Hcy,UA and hs-CRP and the IMT was analyzed.Results The levels of serum Hcy,UA and hs-CRP and the IMT value in LI patients were higher than those in healthy people(P<0.05).As the degree of CAS increased in LI patients,the levels of serum Hcy,UA and hs-CRP were increased(P<0.05).Pearson product-moment analysis showed that the serum levels of Hcy,UA and hs-CRP were significantly positively correlated with the IMT in LI patients(P<0.05).Multiple linear regression showed that Hcy,UA and hs-CRP all were important risk factors for IMT in LI patients(P<0.05).Conclusion The serum levels of Hcy,UA and hs-CRP in patients with LI are significantly increased,and they are positively correlated with carotid IMT significantly.They are important risk factors leading to increase of CAS severity and may increase the risk of LI.
