AIM:To investigate the efficacy and safety of transarterial chemoembolization(TACE)-based multimodal treatment in patients with large hepatocellular carcinoma(HCC).METHODS:A total of 146 consecutive patients were incl...AIM:To investigate the efficacy and safety of transarterial chemoembolization(TACE)-based multimodal treatment in patients with large hepatocellular carcinoma(HCC).METHODS:A total of 146 consecutive patients were included in the analysis,and their medical records and radiological data were reviewed retrospectively.RESULTS:In total,119 patients received TACE-based multi-modal treatments,and the remaining 27 received conservative management.Overall survival(P<0.001)and objective tumor response(P=0.003)were significantly better in the treatment group than in the conservative group.After subgroup analysis,survival benefits were observed not only in the multi-modal treatment group compared with the TACE-only group(P=0.002)but also in the surgical treatment group compared with the loco-regional treatment-only group(P<0.001).Multivariate analysis identified tumor stage(P<0.001)and tumor type(P=0.009)as two independent pre-treatment factors for survival.After adjusting for significant pre-treatment prognostic factors,objective response(P<0.001),surgical treatment(P=0.009),and multi-modal treatment(P=0.002)were identified as independent post-treatment prognostic factors.CONCLUSION:TACE-based multi-modal treatments were safe and more beneficial than conservative management.Salvage surgery after successful downstaging resultedin long-term survival in patients with large,unresectable HCC.展开更多

However, it remains challenging to simultaneously achieve fast electron transport and e...As an im portant branch of fiber-shaped energy storage devices, the fiber-shaped supercapacitor has been widely studied recently. However, it remains challenging to simultaneously achieve fast electron transport and excellent ion accessibility in one single fiber electrode of the fibershaped supercapacitor. Herein, a novel family of amphiphilic core-sheath structured carbon nanotube composite fibers has been developed and applied to the fiber-shaped supercapacitor to address the above challenge. The polyaniline-modified hydrophilic sheath of the composite fiber electrode effectively enhanced the electrochemical property via advancing ion accessibility, while Au-deposited hydrophobic core demonstrated improved electrical conductivity by fast electron supply. On the basis of a synergistic effect, a remarkable specific capacitance of 324 F cm^-3 at 0.5 A cm^-3 and greatly enhanced rate performance were achieved, i.e” a 79% retention (256 F cm 3) at 50 A cm^-3. The obtained fiber-shaped supercapacitor finally displayed remarkable energy and power densities of 7.2 mW h cm 3 and 10 W cm^-3, respectively. The strategy developed herein also presents a general pathway towards novel fiber electrodes for high-performance wearable devices.展开更多

Global industrialization has been a contributor to morbid obesity ...In the last 30 years, operative, technical and medical advances have made liver transplantation(LT) a lifesaving therapy that is used worldwide today. Global industrialization has been a contributor to morbid obesity and this has brought about the metabolic syndrome along with many downstream complications of such. Non-alcoholic steatohepatitis(NASH) has become a recognized hepatic manifestation of the metabolic syndrome and NASH cirrhosis is predicted to be the primary indication for LT in the United States by 2025. Several case series and database reviews have begun analyzing the efficacy of weight reduction surgery in the LT recipient. These data have reasonably demonstrated that weight reduction surgery in the LT recipient is a feasible endeavor. However, several questions have been raised regarding the type of weight reduction surgery, timing of surgery in relation to LT, patient and allograft survival and post-LT maintenance of weight loss to name a few. We look forward to a time when weight reduction surgery will work to improve the technical conduct of LT, improve perioperative benchmarks such as blood transfusions, intensive care unit length of stay and help to prevent recurrence of NASH cirrhosis in the medically complicated obese patient. In the meantime, well-designed prospective clinical trials that focus on the issues highlighted will help guide us in the care of these complicated patients who will soon account for the majority of the patients in our clinics.展开更多
In transportation architecture,wayfinding quality is a crucial factor for determining transfer efficiency and level of service.When developing architectural design concepts,designers often employ their visual attentio...In transportation architecture,wayfinding quality is a crucial factor for determining transfer efficiency and level of service.When developing architectural design concepts,designers often employ their visual attention to imagine where passengers will look.A saliency model is a software program that can predict human visual attention.This research examined whether a saliency model or designer visual attention is a good predictor of passenger visual attention during wayfinding in side transportation architecture.Using a remote eye-tracking system,the eye-movements of 29 participants watching 100 still images depicting different indoor seenes of transportation architecture were recorded and transformed into saliency maps to illustrate participants'visual attention.Participants were categorized as either"designers"or"laypeople"based on their architectural design expertise.Similarities were compared among the"designers'"visual attention,saliency model predictions,and"laypeople's"visual attention.The results showed that while the"designers'"visual attention was the best predictor of that of"laypeople",followed by saliency models,a single desig ner's visual attend on was not a good predictor.The divergence in visual attention highlights the limitation of designers in predicting passenger wayfinding behavior and implies that integrating a saliency model in practice can be beneficial for wayfinding design.展开更多

The physicochemical makeup of the cells aff)cts various cellular responses, determines cell-cell interactions and influences cell decisions. Chemical and physical properties diff)r between cells and within cells. Moreover, these properties are subject to dynamic changes in response to environmental signals, which often demand adjustments in the chemical or physical states of intracellular molecules. Indeed, cellular responses such as gene expression rely on the faithful relay of information from the outside to the inside of the cell, a process terrned signal transduction. The signal often traverses a complex path across subcellular spaces with variable physical chemistry, sometimes even influencing it. Understanding the molecular states of such signaling molecules and their intracellular environments is vital to our understanding of the cell. Exploring such intricate spaces is possible today largely because of experimental and theoretical tools. Here, we focus on one tool that is commonly used in chemical physics studies light. We summarize recent work which uses light to both visualize the cellular environment and also control intracel- lular processes along the axis of signal transduction. We highlight recent accomplishments in optical microscopy and optogenetics, an emerging experimental strategy which utilizes light to control the molecular processes in live cells. We believe that optogenetics lends un- precedented spatiotemporal precision to the manipulation of physicochemical properties in biological contexts. We hope to use this work to demonstrate new opportunities for chemical physicists who are interested in pursuing biological and biomedical questions.展开更多

文摘Timothy Mo,a famous British writer whose works have won many important prizes,experiences both Chinese culture and English culture since his childhood.Due to this kind of experience,his works are usually concerned with the issue of cultural conflict.Among these works,Sour Sweet is the most well-known one in China,and it has also been filmed as Soursweet in 1988 with the famous actress Zhang Aijia.In Sour Sweet,Timothy Mo tells a story about a Chinese family’s struggle in the UK.After experiencing a serious of cultural conflict,among the three family members Chen,Lily and Mui,only Lily achieves a delicate balance.After defining the process of establishing oneself in a new environment as a kind of human translation,the article applies AndréLefevere’s theory to study the three characters’different responses to the cultural conflict,analyze how different poetic and different strategies adopted to deal with ideology affect their translation,explain why does or doesn’t he/she translates himself/herself successfully,and finally gets the conclusion:because Lily deals well with the dominant ideological and poetical currents,and makes appropriate choice in face of cultural translatability and untranslatability,so only she can translate herself successfully.

文摘In the last 30 years, operative, technical and medical advances have made liver transplantation(LT) a lifesaving therapy that is used worldwide today. Global industrialization has been a contributor to morbid obesity and this has brought about the metabolic syndrome along with many downstream complications of such. Non-alcoholic steatohepatitis(NASH) has become a recognized hepatic manifestation of the metabolic syndrome and NASH cirrhosis is predicted to be the primary indication for LT in the United States by 2025. Several case series and database reviews have begun analyzing the efficacy of weight reduction surgery in the LT recipient. These data have reasonably demonstrated that weight reduction surgery in the LT recipient is a feasible endeavor. However, several questions have been raised regarding the type of weight reduction surgery, timing of surgery in relation to LT, patient and allograft survival and post-LT maintenance of weight loss to name a few. We look forward to a time when weight reduction surgery will work to improve the technical conduct of LT, improve perioperative benchmarks such as blood transfusions, intensive care unit length of stay and help to prevent recurrence of NASH cirrhosis in the medically complicated obese patient. In the meantime, well-designed prospective clinical trials that focus on the issues highlighted will help guide us in the care of these complicated patients who will soon account for the majority of the patients in our clinics.

文摘In transportation architecture,wayfinding quality is a crucial factor for determining transfer efficiency and level of service.When developing architectural design concepts,designers often employ their visual attention to imagine where passengers will look.A saliency model is a software program that can predict human visual attention.This research examined whether a saliency model or designer visual attention is a good predictor of passenger visual attention during wayfinding in side transportation architecture.Using a remote eye-tracking system,the eye-movements of 29 participants watching 100 still images depicting different indoor seenes of transportation architecture were recorded and transformed into saliency maps to illustrate participants'visual attention.Participants were categorized as either"designers"or"laypeople"based on their architectural design expertise.Similarities were compared among the"designers'"visual attention,saliency model predictions,and"laypeople's"visual attention.The results showed that while the"designers'"visual attention was the best predictor of that of"laypeople",followed by saliency models,a single desig ner's visual attend on was not a good predictor.The divergence in visual attention highlights the limitation of designers in predicting passenger wayfinding behavior and implies that integrating a saliency model in practice can be beneficial for wayfinding design.
文摘Objective.To study expression and localiza tion of Smad4protein,the common-mediator Smad,which is one of intracellular signaling molecules of transforming growth factor-afamily,in rat t estis during postnatal development.Methods.In this study,whole testes were co llected from S.D rats aged3days,7days,14days and28days,and adult respe ctively.We examined the cellular localization and developmental change of Smad 4in rat testis by immunohistochemical ABC method with glucose oxidase-DAB-nic kel enhancement technique;the quantitative analysis of the immunostaining by t he image analysis system;the Smads pro-teins coexistence in the adult rat test is by the double immune staining for CD14-Smad4and Smad2-Smad4;the prote in expression of Smad during rat testicular development by means of Western blo ts.Results.The protein of Smad4was present in rats from3days of age to adul thood,and the im-munolocalization was exclusively localized to the cytoplasm o f Leydig cells with negative nuclei in the in-terstitial tissue at any time po int.No expression was detected in germ cells.The result of image and sta-tis tical analysis showed that generally,there was a tendency that the expression o f Smad4in the testes increased gradually with the rats developing maturation.C onclusion.Our data provide direct evidence for the molecular mechanism of TGF-aaction in rat testes during postnatal development and spermatogenesis of ra ts.
文摘This paper focuses on the dramatic transformation of the inherited historical urban and architectural characteristics of the city of Al-Najafin Iraq,seeking new ways of seeing the possibilities of preserving and revitalizing the historical center of the city.In addition to being one of the most sacred centers for Shia Muslims,Al-Najaf has a rich cultural and architectural heritage.Its architectural characteristics and urban structure not only reflect the history of the city but also stand as evidence of the most important junctures in Islamic history:the martyrdom of Imam AU ibn Abi Talib,who is a central Islamic spiritual personality and the second most important figure after the Prophet Muhammad.Since its estab-Ushment in 750 AD,the formation of Al-Najaf has never separated from its sociopolitical context and Islamic principles,particularly Shia Muslims'beliefs.Today,the city is a destination for more than million Shia Muslim pilgrims each year,and the number is rapidly increasing.However,as the city continues to develop,its architecture and unique urban structure transform dramatically.The cultural and architectural heritage of the city is seriously threatened.Several factors played a crucial role in the formation and the transformation of the city's specific architectural and urban structures.The most influential factors were mainly related to religion,environment,and politics.This paper introduces the main forces behind the transformation of the deep organizational structures of the architecture and urban fabric in Al-Najaf.The question of whether it is still possible to protect and preserve the remaining parts and revitalize the Old City center is also examined.The goal is to identify the major preservation issues and provide possible insights based on successful preservation experiences that address similar issues indifferent contexts.

文摘The train platforming and routing(TPR)problem is to decide train operations within stations after the networkwise train schedules are determined.A feasible TPR plan requires both platform and route conflict-free,where the avoidance of route conflict is controlled according to three interlocking modes.Although the TPR problem is widely studied,none of them did a serious investigation on the optimality impacts of different interlocking modes in the TPR.Therefore,this paper introduced and formulated a space–time version of TPR considering three interlocking modes and subsequentially conducted numerical experiments to analyze the optimality differences under each mode.Based on the experimental findings,engineering practical suggestions are also provided.In summary,the experiment results showed that both route-locking sectional-release and sectional-locking sectionalrelease modes significantly outperform the route-locking route-release mode.And among them,using the routelocking sectional-release mode can bring notable benefits on large stations with high-density volumes while using the sectional-locking sectional-release mode can always provide outstanding outcomes over various station and traffic settings.

文摘Cells are crowded microenvironments filled with macromolecules undergoing constant phys- ical and chemical interactions. The physicochemical makeup of the cells aff)cts various cellular responses, determines cell-cell interactions and influences cell decisions. Chemical and physical properties diff)r between cells and within cells. Moreover, these properties are subject to dynamic changes in response to environmental signals, which often demand adjustments in the chemical or physical states of intracellular molecules. Indeed, cellular responses such as gene expression rely on the faithful relay of information from the outside to the inside of the cell, a process terrned signal transduction. The signal often traverses a complex path across subcellular spaces with variable physical chemistry, sometimes even influencing it. Understanding the molecular states of such signaling molecules and their intracellular environments is vital to our understanding of the cell. Exploring such intricate spaces is possible today largely because of experimental and theoretical tools. Here, we focus on one tool that is commonly used in chemical physics studies light. We summarize recent work which uses light to both visualize the cellular environment and also control intracel- lular processes along the axis of signal transduction. We highlight recent accomplishments in optical microscopy and optogenetics, an emerging experimental strategy which utilizes light to control the molecular processes in live cells. We believe that optogenetics lends un- precedented spatiotemporal precision to the manipulation of physicochemical properties in biological contexts. We hope to use this work to demonstrate new opportunities for chemical physicists who are interested in pursuing biological and biomedical questions.

It is a product, even an expression, of the spiritual personality and the cultural particularity. Translation is a kind of interlingual and intercultural communication. It involves not only the transference from one language into another, but a whole set of extra-linguistic criteria. In translating we are always crossing a greater or lesser barrier or divide.
