AIM:To study the results of stem-cell therapy under a Compassionate-use Program for patients with recurrent anal fistulae.METHODS:Under controlled circumstances,and approved by European and Spanish laws,a Compassionat...AIM:To study the results of stem-cell therapy under a Compassionate-use Program for patients with recurrent anal fistulae.METHODS:Under controlled circumstances,and approved by European and Spanish laws,a Compassionate-use Program allows the use of stem-cell therapy for patients with very complex anal fistulae.Candidates had previously undergone multiple surgical interventions that had failed to resolve the fistulae,and presented symptomatic recurrence.The intervention consisted of limited surgery(with closure of the internal opening),followed by local implant of stem cells in the fistula-tract wall.Autologous expanded adipose-derived stem cells were the main cell type selected for implant.The first evaluation was performed on the 8th postoperative week;outcome was classified as response or partial response.Evaluation one year after the intervention confirmed if complete healing of the fistula was achieved.RESULTS:Ten patients(8 male)with highly recurrent and complex fistulae were treated(mean age:49years,range:28-76 years).Seven cases were nonCrohn’s fistulae,and three were Crohn’s-associated fistulae.Previous surgical attempts ranged from 3to 12.Two patients presented with preoperative incontinence(Wexner scores of 12 and 13 points).After the intervention,six patients showed clinical response on the 8th postoperative week,with a complete cessation of suppuration from the fistula.Three patients presented a partial response,with an evident decrease in suppuration.A year later,six patients(60%)remained healed,with complete reepithelization of the external opening.Postoperative Wexner Scores were 0 in six cases.The two patients with previous incontinence improved their scores from12 to 8 points and from 13 to 5 points.No adverse reactions or complications related to stem-cell therapy were reported during the study period.CONCLUSION:Stem cells are safe and useful for treating anal fistulae.Healing can be achieved in severe cases,sparing fecal incontinence risk,and improving previous scoring.展开更多
In recent years,significant focus has been placed on resilience,with ongoing studies aiming to identify strategies for reducing the negative effects of job stress and protecting nurses from negative psychosocial outco...In recent years,significant focus has been placed on resilience,with ongoing studies aiming to identify strategies for reducing the negative effects of job stress and protecting nurses from negative psychosocial outcomes.Nevertheless,as far as this topic is concerned,no research has yet been conducted in the context of Saudi psychiatric nurses.The aim of this research,therefore,is to determine how Saudi Arabian psychiatric nurses’professional quality of life and psychological resilience relate to one another.To this aim,a cross-sectional study has been performed in Saudi Arabia incorporating a population of 167(a 66.4%response rate)mental health nurses taken from the largest psychiatric hospital in the capital city Riyadh.Data collection was performed using the popular Arabic version of the Connor–Davidson Resilience and the Arabic version of the Professional Quality of Life Scales.To assess the variables related to both Professional Quality of Life and psychological resilience levels,statistical analyses such as Pearson correlation,ANOVA,t-tests,and linear regression were utilized.The findings show compassion satisfaction and burnout to have a moderate inverse relationship(r=−0.502),while compassion satisfaction(CS)and resilience have a moderate positive correlation(r=0.594).Compared to their morning-shift counterparts,nurses working night shifts reported higher mean scores for resilience(72.70),CS(40.20),burnout(24.52),and compassion fatigue(CF)(28.11).Participants with less than five years’experience in a psychiatric clinic had the highest mean resilience score(72.96).Finally,a positive relationship between resilience and compassion satisfaction(CS)was revealed using linear regression analysis(β=0.536,t=7.080,p=0.000).The study reveals significant differences in the scores assigned to resilience and work-life quality based on educational qualifications,shift-work type,and experiences.Burnout and compassion satisfaction(CS)are found to be significantly correlated,as are compassion satisfaction(CS)and r展开更多
Amid the recent global upsurge of studies on the philosophy of emotion, a reinterpretation of traditional Confucian philosophy, especially Mencius's ideas on the siduan (Four Original Sources) of morality, may shed...Amid the recent global upsurge of studies on the philosophy of emotion, a reinterpretation of traditional Confucian philosophy, especially Mencius's ideas on the siduan (Four Original Sources) of morality, may shed new light on the subject in comparative philosophy. This paper probes the structure of moral feeling and reason described by Mencius's from five perspectives: (1) In view of the relationship between feeling and reason, is it better to use the expression "siduan" or the expression "sixin (Four Heart-minds)"? (2) In view of dispositional feeling, what are the four original sources? (3) In view of moral feeling, what are the structural order of the four sources and the corresponding procedure of reasoning of four heart-minds of Humanity? (4) In view of positive feeling, how does moral feeling grow out of the goodness of human nature? (5) What is the global value of Mencius's ideas on human moral feeling? The author concludes that Mencius's thought on moral feeing has a global value and cross-cultural significance, and that Chinese wisdom is more than regional but universally applicable. The structure of moral feeling and reason that Mencius identified is in accordance with the principles ofzhiliang (grasping the two poles of the beginning and the end) and yongzhong (emloying the middle). The principles ofzhiliang and yongzhong are true universal wisdoms of Confucianism, which should be rekindled today. From a practical point of view, "the Way begins from moral feelings," and Confucius and Yah Hui's seeking the simple, virtuous life is an ideal model of emotional well-being.展开更多
