摘要
背景:p53,Rb,p16和p21蛋白是肿瘤相关基因产物,其异常表达在细胞周期的不同阶段直接或间接影响着肿瘤的形成和发展,DNA倍体异常也是肿瘤细胞周期频发事件,而肿瘤细胞DNA倍体与P53,Rb,P16和P21蛋白异常表达关系间的研究鲜见报道。目的:研究流式细胞仪(FCM)检测肿瘤组织DNA倍体与多种癌相关基因蛋白p53,Rb,p16和p21异常表达的意义。设计:以诊断为依据设立对照的前瞻性研究。地点和对象:实验地点:江苏省肿瘤防治研究所,实验对象:30例女性癌症患者,其中卵巢癌10例,乳腺癌20例,平均年龄55岁。干预:对30例卵巢癌和乳腺癌瘤体中心灶进行3点取样,应用FCM检测DNA倍体及p53,Rb,p16和p21异常表达。主要观察指标:肿瘤组织DNA倍体及p53,Rb,p16和p21蛋白阳性细胞百分率。结果:瘤灶多点取样法可检测出90%(27/30)的异倍体;不同倍性的肿瘤普遍存在程度不同地p53,Rb,p21和p16基因蛋白异常表达。96.7%(29/30)的肿瘤存在至少一种以上癌相关基因(p53,Rb,p16,p21)异常表达。结论:DNA异倍体与多种癌相关基因蛋白异常表达均是肿瘤恶性指标,FCM同步检测这些指标,为快速了解肿瘤细胞生物学特性、把握患者临床治疗及预后提供了良好途径。
BACKGROUND:p53,Rb,p16 and p21 proteins are tumor related gene products,which the abnormal expressions in the different phase of cell cycle directly or indirectly affect the genesis and development of tumors.The abnormity of DNA polyploid is also a frequent affair in tumor cell cycle.However,the reports about the relationship between tumorous cell DNA polyploid and the abnormal expressions of p53,Rb,p16 and p21 proteins are rare. OBJECTIVE:To study the significance of the detection of tumor DNA polyploid and the abnormal expressions of multiple tumor related gene proteins p53,Rb,p16 and p21 by flow cytometer(FCM). DESIGN:A prospective control study based on diagnosis was conducted. SETTING and PARTICIPANTS:The study was completed in Jiangsu Institute for Prevention and Cure of Tumor.Subjects were 30 cases of female cancer patients with a mean age of 55 years old,in which 10 cases with carcinoma of ovary and 20 cases with breast carcinoma. INTERVENTIONS:3 point sampling was operated in the central tumor in 30 cases of carcinoma of ovary and breast carcinoma.DNA polyploid and the abnormal expressions of p53,Rb,p16 and p21 were detected by FCM. MAIN OUTCOME MEASURES:The percentage of positive cells containing tumor DNA polyploid and p53,Rb,p16 and p21 protein. RESULTS:90%(27/30) heteroploid was detected by multiple sampling in tumor focal area and the abnormal expressions of gene proteins of p53,Rb,p21 and p16 were widely existed with different extent in different ploidy tumors.96.7%(29/30) tumors had at least one more abnormal expressions of tumor related genes(p53,Rb,p16,p21). CONCLUSION:DNA heteroploid and the abnormal expressions of multiple tumors related gene proteins are the indexes of tumor severity.The synchronized detection of FCM on these indexes provides a good approach to the rapid understanding of tumor cellular biological features and mastering the clinical treatment and prognosis for patients.
