革兰氏阴性杆菌产超广谱β-内酰胺酶菌的耐药分析

摘要 目的 了解 ESBLs的发生率和耐药特点 ,为临床合理用药提供依据。 方法 从住院及门诊病人的尿、粪、痰、血、分泌物等标本中分离的菌株共 2 84株 ,使用法国梅里埃公司的细菌分析仪进行细菌鉴定和药敏检测及纸片扩散法确证试验。 结果 检出 ESBLs菌株 81株 ,检出率 2 8.5 2 % ;产 ESBLs的菌株主要是大肠埃希氏菌、肺炎克雷伯氏菌、铜绿假单胞菌 ;以阿莫西林和复方新诺明耐药率最高 ,达 90 %以上。 结论 对 ESBLs细菌感染应根据药敏结果选择碳青霉烯类、喹诺酮类及 β-内酰胺类 Objective To comprehend the occurrence rate and drug-resistant characteristic of ESBLs and to provide bases for clinical reasonablyadministration. Methods 284 drug-resistant strains were separated fromthe urine,feces,phlegm,blood and secretion of people.The bacteria appraisaland drug sensitivity test were made by bacteria analyzer which was offered France. Results 81 ESBLs strains were detected and the detectable rate is 28.52%.The productive ESBLs strains are mainly Escherichia coli,Klebsiella pneumoniae,Verdigris pseudomonas.The drug-resistant rate of amoxicillin and compound sinomin are best high,up to 90%. Conclusion The carbomycins,quinovoses and inhibiter of β-lactamases etc.antibiotics are selected in order thattreat ESBLs bacteria infection based on drug sensitivity test.
