临床产超广谱β-内酰胺酶菌的分离情况及耐药性分析

摘要 目的研究临床不同标本中产超广谱β-内酰胺酶(ESBLs)的大肠埃希菌和肺炎克雷伯杆菌的检出情况及其耐药特点。方法采用全自动细菌鉴定/药敏分析仪结合双纸片确证法对我院2005—2006年微生物室从各类标本中分离的大肠埃希菌和肺炎克雷伯杆菌523株进行检测。结果大肠埃希菌产ESBLs的发生率为26.1%,肺炎克雷伯杆菌产ESBLs的发生率为23.1%,中段尿、痰、伤口分泌物3种标本中分离的大肠埃希菌、克雷伯杆菌产ESBLs的发生率间差异均无统计学意义(P>0.05);产ESBLs菌在ICU病房所占比例最大,其次为呼吸科病房,儿科病房所占比例最小;产ESBLs菌株对抗菌药物的耐药性较非产ESBLs菌株差异有统计学意义(P<0.05),且呈现多重耐药。目前我院产ESBLs细菌主要为大肠埃希菌和肺炎克雷伯杆菌。结论产ESBLs菌在我院各个病房、不同标本均有发生,对抗菌药物的耐药性明显高于非产ESBLs菌,除亚胺培南外,产ESBLs菌对多种抗菌药物均出现不同程度的耐药性。 Objective To study the conditions of extended spectrum β -lactamase (ESBLs) produced by Escherichia coli and Klebsiella pneumoniae from different samples and their drug - resistance. Methods Totally 523 strains of Klebsiella and Escherichia coli from different clinical samples in our hospital from 2005 to 2006 were tested with auto - bacteria detection and drugsensitivity analysis instrument and confirmed by methods of double paper. Results The ESBLs - producing trains of Escherichia coli and Klebsiella pneumoniae accounted for 26. 1% and 23. 1% respectively, the isolation rates form the two kinds of samples showed no significant difference ( P 〉 0. 05 ). The isolation rate of ESBLs - producing trains from ICU ward was the highest, next from respiratory ward, and that from pediatrics ward was lowest. The antibiotic resistance of ESBLs -producing trains was obviously higher than that no ESBLs - producing trains, and the resistance was multiple. So far the ESBLs producing trains in our hospital were mainly Escherichia coli and Klebsiella pneumoniae. Conclusion The ESBLs - producing trains come from various wards and different samples. Antibiotic resistance of these trains is significantly higher than that no ESBLs -producing trains. The ESBLs producing trains show drug - resistance against antibiotics in different degrees except for imipenem.
