摘要
目的探讨肩锁关节手术后常见并发症的原因和总结预防对策。方法回顾该类手术后患者83例,根据临床资料分析其发生原因和提出预防对策。结果83例中78例获得随访,术后发生再脱位8例,肩关节疼痛39例,功能障碍8例。结论①肩锁关节再脱位与内固定物松动、断裂,固定时间不充足;喙锁、喙肩韧带修补、重建不理想有关。②肩关节疼痛及功能障碍与术后锻炼不得法及肩关节创伤性炎症有关。③预防最好办法是选用坚强的内固定方式,重视韧带的修补或重建,采用渐进式肩关节功能锻炼。
Objective To study the causes of postoperative complications of the acromioclavicular joint dislocation and summarize the preventive countermeasures. Methods 83 cases who had been treated with surgery for acromioclavicular joint dislocation were reviewed. After their occurrences of the disorder were analyzed according to their clinic information, some prevent countermeasure were proposed. Results 78 cases were followed up for an average of 17 months. After surgery redislocation occurred in 8 cases, 39 cases felt pain at the shoulder joint, and 8 cases experienced dysfunction. Conclusions ①Redislocation of acromioclavicular joint is related with loosening and breakage of the fixators, insufficient internal fixation time and flawed operation. ②Pain and dysfunction are related to the improper operative exercise and the traumatic inflammation. ③The best preventive countermeasures are rigid internal fixation, repair and reconstruction of ligament as well as possible, and step by step functional exercise for the shoulder joint.
