摘要
目的 探讨内痔硬化剂治疗相关性直肠溃疡的内镜特点、演变过程及临床意义。方法 使用PENTAX3 83 0FK电子结肠镜对 2 1例内痔硬化剂治疗相关性直肠溃疡进行结肠镜监测、追踪 ,观察其内镜表现特点及演变过程、治疗效果和愈合情况。结果 本组病灶均为单发 ,且与硬化剂治疗部位相一致 ,和齿状线相连 ,溃疡边界清楚 ,局部较僵硬 ,隆起型者肠腔不同程度狭窄 ,酷似直肠癌镜下改变 ,但病灶不环腔 ,愈合过程类似消化性溃疡 ,治疗情况下愈合所需时间为 4～ 11个月 ,平均 6个月。结论 内痔硬化剂治疗相关性直肠溃疡有其内镜表现特点 ,须与孤立性直肠溃疡综合征、直肠癌等鉴别 ,其愈合所需时间较长 ,通常表现为一种难治性溃疡。
Objective To study the endoscopic characteristics, the development and the clinical significance of internal hemorrhoid hardener treatment correlative rectum ulcer. Methods A PENTAX3830FK electronic colonoscope was used to trace and monitor the endoscopic manifestation characteristics, development, treatment efficacy and healing state in 21 patients correlative rectum ulcera treated by internal hemorrhoid hardener. Results This group of foci are all solitary, the foci was consistent to the treated parts, connected with the dentate line, having clear edge with local stiff part, the prominence ulcera with stenosis intestinal cavity in different degree, exactly like the rectal carcinoma changes under endoscope, but the foci without circular nidi; the healing process like the digestive ulcer. Under treatment, the healing time needed 4～11 months, the mean time was 6 months. Conclusion The internal hemorrhoid hardener treatment correlative ulcer has the endoscopic manifestation characteristics, it must be differentiated with solitary rectal rectumulcer syndrome, rectal carcinoma, et al. It needed a long time to be healed, and turned to a kind of refractory ulcer.
