摘要
目的 :通过磁共振电影 (Cine MRI)动态观察心肌活动特点 ,并结合心功能分析探讨无创性MR在老年心功能评价中的临床应用价值。方法 :使用德国Siemems公司 1.5TeslaMR扫描仪 ,ARGUS软件对 17例老年冠心病患者 ,5例肥厚型心肌病患者 ,8例健康志愿者进行研究。结果 :17例冠心病患者电影磁共振 (Cine -MR)与超声 (US)对左心室舒张、收缩末期容积、每搏输出量和射血分数四项指标测量值接近 ,r界于 0 .6 790～ 0 .92 35之间 (P <0 .0 5 ) ,相关良好 ,其中超声的射血分数稍大。 5例肥厚型心肌病患者 ,按照Maron分型 ,Ⅱ型 2例 ,Ⅲ型 3例 ,表现为每搏输出量下降 ,而射血分数不下降 ,与此相对应的是老年冠心病患者每搏输出量和射血分数均下降较明显。结论 :MRI对于老年心功能评价显示了良好的前景。
Objective:To evaluate the biomechanical dynamic of the heart and elder cardiac function with noninvasive cine-MRI.Methods:1.5 Tesla MR (Siemems Company) with ARGUS software was used on 17 cases with elder coronary heart disease (CHD) 5 cases, hypertrophic cardiomyopathy (HCM) and 8 volunteers.Results:17 cases of CHD examed with cine-MR and ultrasound had good relationalship on End-diastolic volume,end-systolic volume,stroke volume (SV) and ejection fraction (EF),while the r value changed from 0.6790 to 0.9235 (P<0.05) and EF of ultrasound was little more than MR.In 5 cases with HCM,there was 2 cases with Maron styleⅡ,3 cases of style Ⅲ.While SV were decreased in both CHD and HCM ,EF decreased only in CHD ,normal in HCM.Conclusion:MRI shows promise as a new method for research of elder cardiac function.
