年轻急性心肌梗死患者血浆同型半胱氨酸水平测定的意义

摘要 目的 通过测量年轻人急性心肌梗死患者血浆同型半胱氨酸 (HCY)水平的变化 ,探讨其发病机理 ,为预防和治疗提供理论依据。方法 应用高效液相色谱分析法测定血浆HCY浓度。结果 各组内不同性别之间HCY水平无明显差异。与年轻对照组相比 ,年轻人AMI组血浆HCY水平明显增高 ,两组间具有显著性差异 (P <0 0 5 )。年轻人AMI组中不同病因间比较 ,显示冠状动脉粥样硬化组较其他原因组的血浆HCY水平高 ,两组间具有显著性差异(P <0 0 1)。与中老年AMI组相比 ,年轻人AMI组血浆HCY水平较低 ,两组间具有差异 (P <0 0 1) ,但年轻人AMI中冠状动脉粥样硬化组与中老年AMI组相比 ,两组无明显差异 (P >0 0 5 )。结论 HCY与由冠状动脉粥样硬化所致的年轻人AMI密切相关 。 Objectives By measuring the plasma concentration of the homocysteine to investigate the mechanism of acute myocardial infarction (AMI) in young (<40 years old) so as to provide a basis for its prevention and treatment. Methods High performance liquid chromatography was used to examine plasma levels of HCY of 41 young AMI, 50 cases of' old AMI and 40 cases of young control group. Results Plasma HCY was no significant different about sex in all groups. Compare with control group, HCY was obviously difference in patients with young AMI(P<0.05). Among different etiological factor, plasma HCY in coronary atherosclerosis group was significantly higher than other groups. Plasma HCY was detected in patients with old AMI. Compared with the young AMI group, there was also a significant difference(P<0.01), but among the coronary atherosclerosis group, there was no difference between the young AMI group and old AMI group(P>0.05). Conclusions The plasma level of CHY increased obviously in young AMI patients with coronary atherosclerosis, but this improvement is not dependent on other etiological factors.
