摘要
目的 探讨乳腺浸润性导管癌 (IDC)钼靶X线表现与雌激素受体 (ER)水平之间的关系 ,评价根据IDC钼靶X线征象预测ER表达的可行性。资料与方法 181例经乳腺钼靶X线摄影、手术病理证实的IDC ,术后标本经免疫组织化学染色判断ER表达情况 ,将IDC钼靶X线表现中的恶性钙化、毛刺征与免疫组织化学染色所得的ER表达情况进行对比研究。结果 181例IDC中 ,肿块伴毛刺者 85例 ,有钙化者 74例 ;ER阳性者 99例 ,毛刺组ER阳性率与无毛刺组相比有显著性差异 (P <0 .0 1) ,且毛刺组中ER阳性表达更多见于较小癌灶 ;无钙化组ER阳性率较钙化组略高 ,但无统计学意义。结论 IDC中毛刺征与ER表达之间存在相关性 。
Objective To investigate the relationship between the mammographic features and the estrogen receptor (ER) level in infiltrating ductal cancer (IDC) of the breast.Materials and Methods A total of 181 patients with IDC, confirmed by both molybdenum palladium roentgenograph of breast and pathology, was enrolled in this study. ER expression of the mastectomy specimens was determined with immunohistochemical staining technique. The correlation between the malignant calcification and spicule sign seen on mammographs with ER expression was studied.Results Of 181 cases, spicule sign was seen in 85, of which calcification was present in 74, and ER positive expression was seen in 99. Significant difference in ER positive rate existed between lesions with spicule sign and lesions without spicule sign (P<0.01). In the group showing spicule sign, ER positive expression was more frequently seen in patients with small lesions. ER positive rate in lesions without calcifications was slightly higher than that in lesions with calcifications, although no statistically significant difference was found.Conclusion A certain correlation exists between spicule sign and ER expression in patients with IDC. It is feasible to predict ER expression of IDC by molybdenum palladium roentgenographic signs.
