摘要
目的 探讨氨茶碱治疗哮喘的抗炎作用机理。方法 采集 2 0例缓解期过敏性哮喘患儿PBMCs与氨茶碱共同培养 ,按不同浓度 (0、5、1 0、2 0mg/L)分为哮喘对照组 (ACG)、处理组 1 (ATG1 )、ATG2和ATG3,同时设立正常对照组 (NCG)。采用ELISA检测培养 2 4h后各组PBMCs上清液中白细胞介素 4 (IL 4 )、γ 干扰素 (IFN γ)浓度和血清IgE浓度 ,采用ELISPOT检测培养 5h后PBM Cs内合成IL 4和IFN γ的阳性细胞百分比 ,同时作外周血EOS计数。结果 哮喘缓解期患儿静脉血EOS和血清IgE水平均明显高于NCG(P <0 .0 1和P <0 .0 5 ) ;ACGPBMCs合成和分泌IL 4明显高于NCG ,而合成和分泌IFN γ明显低于NCG ,IL 4 /IFN γ比值和IL 4阳性细胞 /IFN γ阳性细胞比值明显高于NCG ;2 0mg/L氨茶碱能显著抑制PBMCs合成和分泌IL 4 ,促进IFN γ的分泌 ,显著降低IL 4 /IFN γ比值和IL 4阳性细胞 /IFN γ阳性细胞比值。结论 哮喘缓解期患儿仍然存在TH2 /TH1亚群失衡 ,2 0mg/L氨茶碱能纠正TH1和TH2细胞因子的失衡状态 。
Objective To explore the mechanism of anti inflammatory of aminophylline in treatment of asthma.Methods PBMCs were separated from vein blood of 20 asthmatic children,divided into four groups: asthma control group (ACG),aminophylline treatment group (ATG)1,ATG2 and ATG3 according to aminophylline concentration (0,5,10 and 20 mg/L).At the same time,normal control group (NCG) including 11 normal children' PBMCs was set up.The concentration of IL 4 and IFN γ in culture supernatant of PBMCs with PMA ionomycin stimulated and cultivated for 24h were measured by ELISA.IL 4 + and IFN γ + cell percentage of PBMCs with PMA ionomycin monesin stimulated and cultivated for 5h were detected by ELISPOT.Results EOS and IgE in ACG were significantly higher than NCG;IL 4 level in ACG was higher significantly than NCG( P <0.01),while IFN γ level was significantly lower than NCG ( t =4.257, P <0.001);IL 4 positive cell percentage in ACG was higher than NCG ( t = 2.174 ,P< 0.05 ),while IFN γ positive cell percentage was lower than NCG ( t =2.437, P <0.05);IL 4/IFN γ ratio and IL 4 + cell/IFN γ + cell ratio in ACG were signifantly higher than NCG,Aminophylline reduced IL 4/IFN γ ratio (0.13±0.07,0.11±0.06, 0.10 ±0.05 and 0.07±0.03 from ACG to ATG3) and IL 4 + cell/IFN γ + cell ratio (4.13±2.58,3.77±2.30,2.98±2.13 and 1.8±0.96 by turns),but only significantly in aminophylline concentration of 20 mg/L( P <0.01 and P <0.05).Conclusions There is T H2 subsets hyperfunction during asthma remission period,characterized by high IL 4 level,IL 4 positive cell percentage,IL 4/IFN γ ratio and IL 4 + cell/IFN γ + cell ratio.These data suggest that cytokines expressed by T H2 are inhibited by therapeutic concentration superior limit (20mg/L) of aminophylline,and its suppression to T H2 cytokines is likely to be an important mechanism of anti inflammation at the therapeutic concentration of aminophylline in asthma.
