摘要
在32具成人尸体上,经过解剖观察和墨汁灌注透明显示,对颈胸部皮肤的动脉血供和静脉回流模式进行了归纳。颈胸部皮瓣动脉较分散,沿胸锁乳突肌两缘纵列;静脉较集中。向锁骨上窝的中心汇聚。根据血供模式,设计了颈胸部两种超长随意皮瓣。充分利用胸锁乳突肌两缘的动脉,设计了薄型超长皮瓣,以乳头下静脉网为回流途径:利用知名的颈外静脉干为回流途径,设计了全厚超长皮瓣。上述两类皮瓣设计,经临床应用,效果满意。
The arterial supply and venousdrainage of a new kind of skin flaps,theextra-long random flaps in the cervi-co-thoracic region were studied in 32cadavers by means of black ink inje-ction technique.The arteries in thisregion were found to be quite disper-sive,while the veins showed a conce-ntrative tendency towards the centreof supraclavicular fossa.Based uponthe principle of blood supply,two ki-nds of extra-long and thin flaps inthis region are suggested:(1)extra-long and thin flaps nourished by thecutaneous arteries emerging from bothmargins of sternocleidomastoid muscleand the subdermal arterial rete,anddrained by the subpapillary venous rete;(2)extra-long and full-thickness ra-ndom flaps drained by external jugularvein.Both kinds of flaps have beenclinically used with satisfactory resu-lts.
