摘要
采用生物力学方法研究各种减压术对胸腰段脊柱稳定性的影响。结果表明:椎板减压术组抗弯强度降低最明显,约为对照组的1/3,屈伸活动则增加1倍;侧前方减压术使屈曲状态下水平位移及旋转角均增加,而抗压及抗弯强度降低;侧方减压术使水平位移及旋转角明显增加并使抗压强度降低最明显。实验结果提示各种减压木有引起胸腰段脊柱失稳的趋势,临床应认真选择减压水井采用适当措施重建胸腰段脊柱稳定。
The effects of three types of decompression on the stabilization of thoracolumbar spines were studied by biomechanics. The results were; (1)The range of flexion-extension were gradually increased in both lateral and posterior decompression groups. (2)The horizontal displacements and rotations along sagital plane of superior vertebra were increased, compression and bending strength were decreased in anterolateral and posterior decompression groups. We concluded that decompression operations could weak the stability of thoracolumbar spines anatomically and cause a tendness of displacement either in horizontal or sagittal plane between vertebrae. Stability reconstructions of spines were necessary.
