摘要
目的探讨超声监测在胸腰椎骨折后路减压术中的应用效果。方法对14例胸腰椎爆裂性骨折患者在后路减压术中应用超声扫描,了解脊髓前方减压和骨块复位,对术前、术后CT扫描结果进行比较,评估减压情况。结果 14例术中均达到了硬膜囊前方满意减压,术后CT复查椎管内骨块复位满意。患者均获得门诊随访,时间2～12个月。神经功能除A级1例无改善外,余均有1～2级的恢复。结论超声监测下可辨认出硬膜囊前方的致压物位置,可获得减压后客观的超声影像图像,能实时提供术者脊髓前方减压情况。
Objective To evaluate the clinical effect of intraoperative ultrasound monitoring of thoracolumbar fractures through posterior decompression.Methods 14 cases with thoracolumbar burst fractures were undergone posterior decompression with ultrasound scanning for spinal cord anterior decompression and fracture reposition.The ultrasound scanning was compared with the preoperative and postoperative CT scan results to evaluate the results.Results 14 cases achieved dural sac satisfactory decompression,and postoperative CT scans confirmed the spinal bone reposition satisfied.All cases were followed up for 2-12 months.Neurological function was improved 1 to 2 grades except 1 grade A patient without improvement.Conclusions The ultrasound monitoring is recognizable for pressure of the dural sac in front of the location.Ultrasound images can be obtained postoperatively.Ultrasound imaging as a real-time method intraoperatively provides the surgeon with spinal cord anterior decompression information.
