摘要
目的 :探讨天鹅型记忆接骨器 (SMC)内固定对骨折愈合过程中 TGF- β表达的影响。 方法 :取新西兰大白兔 35只 ,建立兔双侧肱骨骨折后 SMC和动力加压接骨板 (DCP)内固定模型 ,分别于术后 1、3、7、1 4、2 1、2 8和 5 6 d各处死 5只兔 ,在骨折局部取材 ,应用免疫组化 En Vision法进行标本中 TGF- β定位检测 ,并结合计算机图像分析系统检测骨折局部 TGF- β的阳性染色强度、阳性面积及阳性指数。结果 :SMC和 DCP固定后骨折愈合过程中 ,间充质细胞、成骨细胞、软骨细胞及基质中均有 TGF- β阳性染色。骨折早期 ,两组 TGF- β含量无显著差异 ;术后 7~ 2 8d,SMC组 TGF- β含量明显高于 DCP组 (P<0 .0 5 ,P<0 .0 1 ) ;术后 5 6 d两组间 TGF- β含量无差异。 结论 :在软骨形成阶段和软骨内成骨阶段 ,SMC内固定下骨折端 TGF- β呈高水平表达 ,从而促进骨折愈合 ,与 SMC所产生的持续动态加压应力有关。
Objective:To study the effect of internal fixation with swan like memory compressive connector(SMC) on temporal expression patterns of TGF β at fracture site. Methods:Double sided humeral fracture in 35 New Zealand rabbits were internally fixed by SMC or dynamic compression plate(DCP) severally. Animals were killed in group of 5 at different time points(1,3,7,14,21,28,56 d post operatively), and immunohistochemistry(EnVision) combined with computer image analysis system was used to examine distribution and density of TGF β at fracture sites. Results:The TGF β staining was localized to the mesenchymal cells, osteoblasts, chondrocytes and extracellular matrix during fracture healing. At the early and late stage after operation, no significant differences of local TGF β content were found between the 2 groups. From day 7 to day 28, the expression of local TGF β in SMC group was significantly stronger than that in DCP group. Conclusion:The sustained dynamic memory compressive stress produced by SMC may enhance the biosynthesis and excretion of TGF β at fracture sites, thus promote bone healing.
