摘要 目的 探讨血管紧张素 (Ang)II受体拮抗剂氯沙坦在治疗心功能不全中的临床疗效。方法 选择NYHA心功能分级为II~IV级、左室射血分数 (LVEF)≤ 5 0 %的患者 6 8例 ,随机单盲分为氯沙坦和福辛普利组 ,于服药前及服药 6个月后观察临床症状、超声心功能图。结果  2组用药后左室射血分数 (LVEF)、心输出量 (CO)、心脏指数 (CI)及左室舒张末内径 (LVEDD)等指标均有改善 (均为P <0 .0 5 ) ;氯沙坦组血尿酸 (UA)水平降低 (P <0 .0 5 ) ,而对照组无明显变化 (P >0 .0 5 ) ;福辛普利组 4例出现干咳 ,而氯沙坦组无干咳病例出现。结论 氯沙坦和福辛普利均能恢复左心室收缩、舒张功能 ,但氯沙坦可降低血尿酸水平 ,且耐受性好。 Objective To observe the therapeutic effect of losartan on cardiac insufficiency.Methods 68 patients with cardiac insufficiency whose heart function was classified as NYHA function class II~IV and LVEF ≤50% were randomly divided into losartan group and fosinopril group. The observation of clinical symptoms and echocardiagraphic function measurements were performed before and 6 months after treatment. Results After treatment, the LVEF,CO,CI and LVEDD were improved obviously, as compared with those before treatment (P<0.05);the levels of UA in losartan group were decreased significantly (P<0.05),however,which was not changed in fosinopril group (P>0.05);there were 4 patients with dry cough in fosinopril group, however, no case in losartan group.Conclusion Both losartan and fosinopril can improve LV systolic and diastolic function, but losartan can reduce the levels of UA with good tolerance.
