吸入用丙酸倍氯米松辅助阿奇霉素序贯法治疗支原体肺炎的疗效

摘要 目的 :分析吸入用丙酸倍氯米松和阿奇霉素序贯法治疗小儿支原体肺炎的临床疗效及对患儿炎症因子的影响。方法 :选取2016年12月—2017年12月我院收治的110例支原体肺炎患儿作为研究对象,采用随机数字表法分为对照组和观察组,各55例,对照组采用阿奇霉素序贯法(阿奇霉素的使用方法为10 mg/kg,按照每10 mg阿奇霉素给予10 mL 0.9%的氯化钠注射液的比例稀释,静脉滴注,qd,持续使用3天后停止用药;休息4 d后,再给予阿奇霉素颗粒10 mg/kg,qd)治疗,观察组在对照组基础上给予吸入用丙酸倍氯米松(0.4 mg加入2 mL 0.9%氯化钠注射液稀释,雾化吸入,早、晚各1次)辅助治疗,比较两组临床疗效、治疗前后炎症因子变化、相关症状消失时间以及不良反应发生情况。结果 :观察组总有效率高于对照组,差异有统计学意义(P <0.05);治疗后,两组超敏C反应蛋白(hs-CRP)、肿瘤坏死因子(TNF)-α和白介素(IL)-6水平较治疗前显著下降,且观察组低于对照组,差异有统计学意义(P <0.05);观察组退热时间、咳嗽及肺部啰音消失时间、住院时间均短于对照组,差异有统计学意义(P <0.05);观察组不良反应总发生率低于对照组,差异有统计学意义(P <0.05)。结论 :吸入用丙酸倍氯米松辅助阿奇霉素序贯法治疗小儿支原体肺炎的疗效显著,可降低炎症因子水平,缓解临床症状,减少住院时间,且安全性好,值得推广使用。 Objective: To analyze the clinical efficacy of beclomethasone dipropionate for inhalation assisted with azithromycin sequential therapy in the treatment of mycoplasmal pneumonia and its effect on airway inflammatory factors in children. Methods: 110 children with mycoplasmal pneumonia admitted to our hospital from December 2016 to December 2017 were selected as subjects investigated and randomly divided into a control group and an observation group, 55 cases in each group. The children in the control group were given azithromycin sequential therapy(The dosage of azithromycin was 10 mg/kg, which was used after azithromycin per 10 mg was diluted by 10 mL of 0.9% sodium chloride solution, intravenous drip, qd, stopping medication after 3 days of sustained use;after 4 days of rest, azithromycin granules 10 mg/kg, qd were given), and the children in the observation group were given beclomethasone dipropionate for inhalation(0.4 mg drug was diluted with 2 mL 0.9% sodium chloride injection, aerosol inhalation, once in morning and evening, respectively) on the basis of the control group. The clinical efficacy, changes in inflammatory factors before and after treatment, disappearance time of related symptoms and occurrence of adverse reactions were compared between the two groups. Results: The total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant(P<0.05). The levels of high-sensitivity C-reactive protein(hs-CRP), tumor necrosis factor(TNF)-α and interleukin(IL)-6 in the two groups after treatment were significantly lower than those before treatment, and the levels of them in the observation group were higher than those in the confrol group, the differences were statistically significant(P<0.05). The disappearance time of fever, cough and lung rale and hospitalization time in the observation group were shorter than those in the control group, and the differences were statistically significant(P<0.05).
